Leishmaniasis at the end of the millennium

Abstract: The term leishmaniasis refers to the spectrum of disease caused by Leishmania species. The clinical manifestations of leishmaniasis depend on complex interactions between the virulence characteristics of the infecting Leishmania species and the genetically determined cell-mediated immune responses of its mammalian host. Leishmaniasis comprises three major syndromes: cutaneous, mucosal, and visceral. Variations exist within each syndrome.

“…Pentavalent antimonial compounds were considered the treatment of choice for decades, but the development of resistance among L. donovani isolates in countries such as India and serious side effects have limited their use in many areas [Berman, 2005; Herwaldt et al, 1992]. Even so, it is still effective and remains standard therapy in many areas of the world, including Brazil [Pearson et al, 1999; Murray et al, 2005]. The recommended dose is 20 mg pentavalent antimony/kg body weight per day given intravenously or intramuscularly.…”

“…Leishmaniasis is a vector-borne parasitic disease endemic in nearly 100 countries worldwide (8,33). The disease is contracted when an infected female phlebotamine sandfly takes a blood meal, injecting promastigotes into the skin of the host.…”

“…transform into the amastigote stage to survive and replicate in macrophage phagolysosomes (36). Clinical manifestations of the disease range from localized, self-healing skin lesions to a potentially fatal disease where parasites disseminate to the visceral organs (8,33). Leishmania major causes cutaneous leishmaniasis.…”

“…This is a particularly important factor in endemic regions as observation may be appropriate if the lesion is not located in a cos‐metically sensitive site [2, 3]. Because precise speciation is essential for prognosis and therapy, a sample biopsy should be taken to confirm infection and PCR performed for speciation [4, 5].…”
