INTRODUCTION Total hip replacement (THR) is successful and performed commonly. Component placement is a determinant of outcome. Influence of surgeon handedness on component placement has not been considered previously. This study was a radiographic assessment of component positioning with respect to handedness. Early data from 160 patients are reported. METHODS Overall, 160 primary THRs for osteoarthritis were included. Equal numbers of left and right THRs were performed by four surgeons, two right-handed and two left-handed. Postoperative radiography was assessed for THR component position by measurement of leg length inequality, acetabular inclination and centre of rotation. Surgeons' handedness was assessed using the Edinburgh inventory. RESULTS For leg length inequality, no significant interaction was seen between hip side and surgeon handedness. Acetabular inclination angles showed a statistically significant difference, however, depending on hand dominance, with higher inclination angles recorded when operating on the dominant side. There was a trend towards greater medialisation of the centre of rotation on the non-dominant side although this did not reach statistical significance. CONCLUSIONS Surgeon handedness appears to influence acetabular component position during THR but it is one factor of many that interact to achieve a successful outcome.

BackgroundSymptomatic leg length inequality accounts for 8.7% of total hip replacement related claims made against the UK National Health Service Litigation authority. It has not been established whether symptomatic leg length inequality patients following total hip replacement have abnormal hip kinetics during gait.MethodsHip kinetics in 15 unilateral total hip replacement patients with symptomatic leg length inequality during gait was determined through multibody dynamics and compared to 15 native hip healthy controls and 15 ‘successful’ asymptomatic unilateral total hip replacement patients.FindingMore significant differences from normal were found in symptomatic leg length inequality patients than in asymptomatic total hip replacement patients. The leg length inequality patients had altered functions defined by lower gait velocity, reduced stride length, reduced ground reaction force, decreased hip range of motion, reduced hip moment and less dynamic hip force with a 24% lower heel-strike peak, 66% higher mid-stance trough and 37% lower toe-off peak. Greater asymmetry in hip contact force was also observed in leg length inequality patients.InterpretationThese gait adaptions may affect the function of the implant and other healthy joints in symptomatic leg length inequality patients. This study provides important information for the musculoskeletal function and rehabilitation of symptomatic leg length inequality patients.

Робота присвячена вивченню основних напрямків досліджень біомеханічних особливостей ходьби людини після тотального ендопротезування кульшового суглоба в історичному аспекті й сучасного стану проблеми. Особлива увага звертається на проблему несиметричності довжини важеля абдукторів стегна після протезування, а саме наслідки, до яких призводить ця помилка. Розглянуті методи досліджень біомеханіки ходьби людини, як теоретичні, зокрема математичне динамічне моделювання, так і апаратні. Аналіз літератури показав, що при тяжких дегенеративно-дистрофічних ураженнях кульшового суглоба у хворих відбувається тяжке спотворення біомеханіки ходьби, що виражається в зменшені часу опори на стопу, переносі ваги тіла на здорову чи порівняно здорову кінцівку, зменшенні довжини кроку через виражену контрактуру в кульшовому суглобі. У хворих спостерігається зростання асиметрії кроків, що робляться здоровою й хворою кінцівками, за різними параметрами. Часто спостерігається нестабільність параметрів від кроку до кроку. Ендопротезування кульшового суглоба приводить до позитивних зсувів параметрів ходьби, а саме збільшення довжини кроку, відновлення опори на протезовану кінцівку. Зростає швидкість пересування. Але в більшості хворих зберігається недостатність м’язового апарату, який під час перебігу захворювання втратив спроможність повноцінного функціонування. Крім того, проведене хірургічне втручання теж впливає на якість ходьби. За даними літератури було виявлено, що асиметрія довжини важеля абдукторів стегна відіграє свою негативну роль в уповільнені відновлення ходьби, особливо у віддалені періоди.
