BACKGROUND-Previous studies relying on clinical care data have suggested that atrial fibrillation is less common in African Americans than Caucasians, but the mechanism remains unknown. Clinical care may itself vary by race, potentially affecting the accuracy of atrial fibrillation ascertainment in studies relying on clinical data. We sought to examine racial differences in atrial fibrillation prevalence determined by protocol-driven electrocardiograms (ECGs) obtained in prospective cohort studies and to study racial differences in echocardiographic characteristics.

Аритмии широко распространены в организован-ных и неорганизованных популяциях, встречаются как при различных заболеваниях, так и у здоровых людей, наиболее часто развиваются при наличии структурных изменений сердца, но могут встречаться и в их от-сутствие. Выявление предикторов аритмий является важной задачей, необходимой для раннего начала лечения и уменьшения его стоимости, проведения про-филактических мероприятий, которые могут улучшить прогноз (снизить риск заболеваемости, смертности, инсульта) [1]. К настоящему времени выполнены многочисленные исследования, посвященные изуче-нию аритмий, с использованием суточного монито-рирования ЭКГ по Холтеру (ХМЭКГ) и эхокардиогра-фии [2][3][4]. Целью нашего исследования было изуче-ние ассоциации отдельных видов аритмий с откло-нениями эхокардиографических параметров от нор-мы у взрослого населения Заполярья, проживающе-го в условиях неблагополучной экологической обста-новки. Цель. Изучить связь аритмий, зарегистрированных при электрокардиографии (ЭКГ) и суточном мониторировании ЭКГ по Холтеру (ХМЭКГ), с выявленными при про-ведении эхокардиографии изменениями структуры и функции сердца у взрослого населения Заполярья, проживающего в условиях неблагополучной экологической обстановки. Материал и методы. Обследовали 348 человек обоего пола в возрасте 20 лет и старше, отобранных методом случайной выборки. Всем участникам исследования выполнялись ЭКГ, ХМЭКГ и трансторакальная эхокардиография. Результаты. Выявлены статистически значимые ассоциации между отдельными видами аритмий и эхокардиографическими изменениями. Установлены корреляции средней силы между фибрилляцией предсердий (ФП) по данным ЭКГ и дилатацией левого (r=0,3215) и правого (r=0,3275) предсердий, между ФП по данным ХМЭКГ и систолической дисфункцией левого желудочка (ЛЖ) (r=0,3216), а также между желудочковой экстрасистолией по данным ХМЭКГ и гипертрофией ЛЖ (r=0,3332). Заключение. Подтверждена ассоциация аритмий со структурными и функциональными изменениями сердца у лиц, проживающих в неблагоприятных природно-климатических условиях. Aim. To study the relationship of arrhythmias, registered by the electrocardiography (ECG) and ECG Holter monitoring, with the changes of the heart and disorders of its function detected by echocardiography in the adult population of the Arctic, living in unfavorable environmental conditions. Material and methods. Patients (n=348) of both sexes, aged 20 years and older, were randomly selected and examined. All participants were carried out ECG recording in 12 standard leads, Holter monitoring and transthoracic echocardiography. Results.
