Lateral Decubitus Positioning Stereotactic Vacuum-Assisted Breast Biopsy with True Lateral Mammography

Abstract: Stereotactic vacuum-assisted breast biopsy (VAB) has been used to evaluate microcalcifications or non-palpable breast lesions on mammography. Although stereotactic VAB is usually performed in a prone or upright position, an expensive prone table is necessary and vasovagal reactions often occur during the procedure. For these reasons, the lateral decubitus position can be applied for stereotactic VAB, and true lateral mammography can be used to detect the lesion. We report on 15 cases of lateral decubitus posit… Show more

“…The prone position, which is used in most institutions, is reliable and accurate but requires significant space and is relatively expensive. Some institutions prefer the sitting or decubitus position, but they can present problems due to the relatively high frequency of vasovagal reactions and patient motion ( 16 18 21 ). Digital breast tomosynthesis-guided biopsies are performed at some institutions using the lateral decubitus position, and a higher biopsy success rate in less procedure time was reported compared with prone S-VAB ( 20 ).…”

“…US-VAB and S-VAB were performed using 8- or 11-gauge vacuum probes (Mammotome; Ethicon Endo-Surgery, Inc., Cincinnati, OH, USA). S-VAB was performed in the lateral decubitus position ( 16 ) with the breast lesion facing up (Digital Stereotaxy with Senographe DS Interventional; GE Healthcare). A microclip was inserted at the biopsy site after S-VAB only.…”

“…The microcalcification would be located at the point at which the two straight lines meet. A true lateral mammograph is sometimes helpful to accurately locate a microcalcification (16). If the approximate location of the microcalcification is obtained, some experts can identify the microcalcification even using US.…”

“…VABB is an accurate and useful method for diagnosing mammographically detected lesions including suspicious microcalcifications [ 6 , 10 , 11 ]. Compared to 14-gauge core needle biopsy (CNB), VABB has an advantage of lower underestimation rate because of the larger amount of calcification retrieval.…”
