China has a large burden of primary angle-closure glaucoma (PACG) and the approach to treatment is different from that in most parts of the world. In China, trabeculectomy is a mainstay of initial treatment for PACG and laser peripheral iridotomy is reserved for PAC and PACG with peripheral angle synechiae ≤180 degrees. The greater reduction of and more stable IOP, lower frequency of acute angle closure, decreased progression of peripheral anterior synechia and glaucoma, and the reduction of ocular complications associated with long-term use of glaucoma medications support the use of initial trabeculectomy in PACG without significant lens opacities. Further study is required to determine the indications and effectiveness of initial trabeculectomy for PACG in a developing country setting.

“…We found that iridoplasty was effective in releasing PAS and there was a significant reduction in the extent of PAS after the iridoplasty procedure, and thus, this procedure may be helpful in cases with PAS at presentation with or without a high IOP. In the study conducted by Sun et al, 26 the authors stated that iridotomy alone released PAS to some extent, but the combined laser technique, iridotomy plus iridoplasty, released more of the PAS. We however did not find that iridotomy alone caused any reduction of PAS, but after iridoplasty, there was a significant decrease in the extent of PAS.…”

In eyes with synechial angle closure and PIS that do not show an improvement after an iridotomy, laser peripheral iridoplasty can be very effective. There were no significant complications after the iridoplasty procedure, implying that it is quite safe. This study also demonstrates that AS-OCT can serve as a useful tool to document the preprocedure angle parameters, to note the changes after the procedure, and for the long-term follow-up of these patients.
