Laryngeal Prostatic Cancer Metastases: An Underestimated Route of Metastases?

Abstract: The larynx is more often affected in metastatic prostatic cancer disease than is suggested by clinical experience. Because prostatic cancer metastasis starts in the hematopoietic areas of the ossified laryngeal skeleton, macroscopic evaluation of the whole larynx fails to detect metastases. Detecting laryngeal involvement requires horizontal slices and histological analysis.

“…As disadvantage of ossification it should be noted that cartilages of the laryngeal skeleton are damaged by laryngeal cancer mainly in ossified and vascularized sites (Olszewski, ; Kirchner, ; Tillmann and Paulsen, ; Paulsen and Tillmann, ). Furthermore, the ossified laryngeal framework is not only destroyed by invading laryngeal cancer cells, but also a target of metastases of primary tumors which prefer bone, such as for example prostatic cancer (Prescher et al, ).…”

“…The larynx is a rare but potential site of prostate carcinoma metastasis [1]. The tumor cells first metastasize to the haematopoietic tissue within the laryngeal cartilages, followed by local destruction and subsequent development of a perilaryngeal soft tissue mass.…”

“…The most frequent primary tumors that metastasize to the larynx include melanoma, renal, colorectal, breast and lung. Prostate carcinoma metastasis to the larynx is rare, with fewer than 20 cases reported [1].…”

“…The tumors which most often metastasize to the larynx are the melanoma and the clear cell renal carcinoma. There are, however, reports of colon, pancreas, breast and prostate cancer metastases 1,3 . The supraglottis is the most affected site by these metastases, and the glottis is the less frequent one Laryngeal involvement by the tumor may or may not cause symptoms, hoarseness and stridor.…”
