125 StreszczenieTradycyjną metodą postępowania w przypadku raka endometrium jest śródoperacyjne płukanie jamy otrzewnej, przeprowadzona na drodze laparotomii całkowita histerektomia, obustronne wycięcie przydatków, a także -w przypadku występowania dużego ryzyka zajęcia przez proces nowotworowy -usunięcie węzłów chłonnych miednicznych i okołoaortalnych. Początkowo alternatywę dla laparotomii w przypadku raka endometrialnego stanowiło wycięcie macicy na drodze pochwowej wspomagane laparoskopowo (laparoscopic-assisted vaginal hysterectomy -LAVH) z limfadenektomią. Obecnie częściej wykonywane jest całkowite laparoskopowe wycięcie macicy (total laparoscopic hysterectomy -TLH) z limfadenektomią.Główną zaletą zastosowania procedury laparoskopowej jest precyzyjna preparatyka, z którą wiąże się mniejsza utrata krwi niż w przypadku laparotomii. Zastosowanie procedury laparoskopowej zmniejsza liczbę śród-i pooperacyjnych powikłań, zwłaszcza takich jak: infekcja rany pooperacyjnej, rozejście się rany pooperacyjnej oraz przepuklina w ranie pooperacyjnej. Dzięki laparoskopii skrócony jest również czas hospitalizacji, co wiąże się przede wszystkim z mniejszym urazem operacyjnym, mniejszą utratą krwi podczas operacji, z mniejszą liczbą powikłań, a także z szybszą rekonwalescencją. Ponadto autorzy licznych publikacji potwierdzają swoimi badaniami brak różnicy statystycznej w liczbie pobranych do badania histopatologicznego węzłów chłonnych podczas wykonywania limfadenektomii miednicznej i okołoaortalnej na drodze laparoskopii i laparotomii, co świadczy o podobnej skuteczności obydwu metod. Dane prezentowane w dostępnych publikacjach wskazują również na porównywalny odsetek 5-letnich przeżyć i nawrotów w przypadku laparoskopii i laparotomii u pacjentek z rakiem endometrium.Pomimo korzyści, jakie płyną z wykonania procedury laparoskopowej w leczeniu raka endometrium, w dalszym ciągu wśród ginekologów pojawiają się obawy i wątpliwości związane z zastosowaniem endoskopii. Przyczyną tego jest fakt, że procedury laparoskopowe stosowane w terapii raka endometrium należą do niezwykle trudnych technik operacyjnych. Związana z tym jest dłuższa krzywa uczenia i dłuższy czas trwania operacji niż w przypadku metody otwartej.Laparoskopia w terapii raka endometrium stanowi procedurę bezpieczną onkologicznie, skuteczną i wykonalną. Słowa kluczowe: rak endometrium, laparoskopia, laparotomia. SummaryAbdominal total hysterectomy with bilateral adnexectomy, intra-operative peritoneal washing, and also lymphadenectomy in the case of a high risk of pelvic and para-aortic nodal metastasis, constitute the traditional method of treatment of endometrial cancer. Two innovative approaches in minimally invasive surgery that have been introduced recently are the da Vinci robotic platform and single port laparoscopic surgery (SPLS). Robotic surgery has many advantages such as 3-dimensional view, the wrist like motion of the robotic arm and ergonomically comfortable position for the surgeon. Numerous literatures have demonstrated the feasibility of robotic surgery in gynecologic oncology. However, further research should be performed to demonstrate the superiority of robotic surgery compared to conventional laparoscopy. Additionally, cost reduction of robotic surgery is needed to adopt robotic surgery into gynecologic oncology worldwide. SPLS has several possible benefits including reduced operative complications, reduced postoperative pain, and better cosmetic results compared to conventional laparoscopy. Although several authors have indicated that SPLS is a feasible approach for gynecologic surgery, there have been few reports demonstrating the potential advantages over conventional laparoscopy. Moreover, technical difficulties of SPLS still exist. Therefore, the advantages of a single port approach compared to conventional laparoscope should be evaluated with comparative study, and further technologic development for SPLS is also needed. These two progressive technologies take the lead in the development of MIS and further studies should be performed to evaluate the benefits of robot surgery and SPLS.

The purpose of this study was to evaluate the surgical feasibility of and survival outcome after laparoscopy in obese Korean women with endometrial cancer which has recently been increasing. We reviewed the medical records of the patients treated at our medical institution between 1999 and 2012. The patients were divided into three groups, nonobese (Body Mass Index [BMI] < 25.0), overweight (BMI 25-27.99), and obese (BMI ≥ 28.0). These patient groups were compared in terms of their clinical characteristics, treatment methods, as well as surgical and survival outcomes. In total, 55 of the 278 eligible patients were obese women. There were no differences in the three groups in terms of the proportion of patients who underwent lymphadenectomy, their cancer stage, histologic type, type of adjuvant treatment administered, intra-, post-operative, and longterm complications, operative time, number of removed lymph nodes, blood loss, and duration of hospitalization (P = 0.067, 0.435, 0.757, 0.739, 0.458, 0.173, 0.076, 0.124, 0.770, 0.739, and 0.831, respectively). The Disease-Free Survival (DFS) times were 139.1 vs. 121.6 vs. 135.5 months (P = 0.313), and the Overall Survival (OS) times were 145.2 vs. 124.8 vs. 139.5 months (P = 0.436) for each group, respectively. Obese women with endometrial cancer can, therefore, be as safely managed using laparoscopy as women with normal BMIs.
