An internal hernia is the protrusion of visceral contents through a congenital or acquired defect in the peritoneum or mesentery within the abdominal cavity. In approximately 0.6-5.8% of patients with small intestinal obstruction, the cause is internal hernia, with paraduodenal hernias accounting for approximately 40% of cases. Here, we present the case of a 51-year-old man diagnosed with obstruction of the small intestine caused by a hernia on the left side of the duodenum. The treatment involved returning the bowel loops to the normal position and closing the hernia pocket using Prolene 2.0 sutures. The duration of the surgery was 30 min. Five days later, the patient's condition was stable and he was discharged from the hospital; at the 32-month postoperative follow-up, he remained in stable condition with no recurrence. An abdominal computed tomography scan is valuable for early diagnosis of paraduodenal hernia in the absence of complications, and the cause can be identified and the bowel returned to the normal position by endoscopic surgery, with closure of the hernia pocket if the intestine does not stick to the pocket.

Introduction:Internal hernia is a relatively uncommon condition and is a rare type of intestinal obstruction. Paraduodenal hernia is considered the most common type of internal hernias. The rare prevalence and the variable symptoms make the clinical diagnosis of paraduodenal hernia a diagnostic challenge. They represent 1% of syndromes that can cause intestinal obstruction.Material and Methods: Retrospective case series of four patients diagnosed and operated in Hamad General Hospital, Doha, Qatar with a follow up period of one year.Results and Discussion: Three patients had left-sided paraduodenal hernia and one patient had right-sided paraduodenal hernia. Three patients were diagnosed using CT scan and one pregnant female was diagnosed with an MRI. Two patients were operated laparoscopically and the other two had open exploration and repair of the hernia. Average hospital stay was 4.75 days. All patients recovered well with an average follow up period of 13 Due to the nonspecific clinical picture; diagnosing paraduodneal hernia requires high index of suspicion. Laparoscopic repair of Para duodenal hernia is safe and can be carried out on elective basis for non-obstructed patients. Surgical intervention is the treatment of choice, also in asymptomatic cases, because it reduces the urgent surgery and hence reducing the morbidity and mortality associated with emergency laparotomy.

A 50-years old woman presented to the Emergency Department after 6 h of abdominal discomfort, pain, bowel distension, nausea and vomiting. She referred acute onset of oppressive pain in left upper quadrant and left flank with painful and tender abdomen. Bowel sounds were present. Rectal examination was unremarkable. Past medical history included hypertension and no previous abdominal surgery. Laboratory tests showed a slight increasing in white blood cell count (10.800/μL) and PCR (15 mg/dL). Computed tomography revealed a cluster of ileal or jejunal loops in the leftupper quadrant (lateral to the ligament of Treitz and posterior to the inferior mesenteric vein (IMV)) with dilated proximal small bowel loops (Fig. 1). The patient was placed in a slight anti-Trendelenburg position with a 10 right rotation. Three operative trocars were used. A laparoscopic exploration confirmed the diagnosis of a left paraduodenal hernia with about 130 cm of viable small bowel incarcerated.
