Laminectomy and fusion vs laminoplasty for multi-level cervical myelopathy: a systematic review and meta-analysis

Abstract: From the available low-quality evidence, LF and EL approaches for CSM demonstrates similar clinical improvement and loss of lordosis. However, a higher complication rate was found in LF group, including significantly higher nerve palsy complications. This requires further validation and investigation in larger sample-size prospective and randomized studies.

“…LP and LF both increased the volume of the spinal canal, relieved spinal cord compression, and did not cause cervical spine instability. Many studies have proven that LF and LP are both safe and effective [3,4,11,17,23]. Our results also show no signi cant difference in the JOA score, VAS-N score, SF-36 MCS or SF-36 PCS between the two groups at the nal follow-up visit.…”

“…LF and LP are the common treatments for MCSM [23]. A number of previous studies have proven that as a treatment option of MCSM, both methods can improve the symptoms and prevent further deterioration of the nervous system with no difference in e cacy [3,11,17,23]. However, ISI on T2WI underwent laminoplasty has been reported to be associated with poor surgical outcomes [10].…”

“…4,6 In a recent systematic review and meta-analysis, clinical outcomes were similar when comparing laminoplasty to laminectomy with fusion, whereas some independent studies have demonstrated superior clinical outcomes and subjective improvement with decreased complications following laminoplasty when performed on a neutral or kyphotic cervical spine. 4,7 Evidence suggests that motion is preserved. Machino et al 5 reported that 87.9% of cervical range of motion was preserved.…”

“…The statistics have been described elsewhere. 20 25 Briefly, the mean difference (MD) and relative risk (RR) were used as a summary statistic. In the present study, both fixed- and random-effect models were tested.…”
