Lactate and liver function tests after living donor right hepatectomy: a comparison of solutions with and without lactate

Abstract: This prospective randomized study showed that non-lactate-containing crystalloid solution may have important advantages over LR solution, concerning lactate and liver profiles, in living donors undergoing right hepatectomy.

“…43 Of particular interest to our findings was a study of 24 patients undergoing open gynecologic surgery randomly assigned to LR or NS; preoperative to postoperative lactate increased from 0.6 mmol/L to 2.0 mmol/L in the LR group, compared with 0.6 mmol/L to 0.4 mmol/L in the NS group (p G 0.05). 10 In addition, Hadimioglu et al 9 performed a double-blinded RCT of renal transplant patients among NS, Plasma-Lyte, and LR and found that lactate significantly increased only in the LR group (0.48 [0.29] mmol/L vs. 1.95 [0.48] mmol/L non-LR vs. LR; p G 0.05) but without a concomitant acidosis.…”

“…A study including 24 healthy volunteers recorded no hyperlactacidemia after infusing a liter of this solution in 1 h. 38 The use of Ringer lactate produced hyperlactacidemia in polytraumatized patients, 26 in gynecological surgery, 32 in surgical patients over 60 years of age, 27 and in donors subjected to right hepatectomy---though it disappeared on the second postoperative day, and had no impact upon the clinical outcome. 39 The relative hypoosmolarity of Ringer lactate constitutes an important limitation for its use in the resuscitation of patients at risk of brain edema. 40 It is therefore not advised in patients with traumatic brain injury or in other neurological patients at risk of developing intracranial hypertension.…”

“…Un estudio que incluyó 24 voluntarios sanos no observó hiperlactacidemia tras infundir un litro de esta solución en una hora 38 . El empleo de Ringer Lactato ocasiona hiperlactacidemia en pacientes politraumatizados 26 , en cirugía ginecológica 32 , pacientes quirúrgicos mayores de 60 años 27 , o donantes sometidos a hepatectomía derecha, si bien la misma desaparecía al segundo día de posoperatorio y no tenía impacto en la evolución clínica 39 .…”
