Laboratory aids to diagnosis—enzymes

Abstract: A summary is presented of those organ specific enzyme assays traditionally used in evaluation of the patient with cancer. In addition, the use of certain serum enzymes such as gamma-glutamyl transpeptidase, phosphohexose isomerase or 5'-nucleotidase as aids in following the course of the disease, particularly in patients with metastatic spread to the liver is outlined. Also considered is the utility of enzyme analysis in biopsy tissue, biologic fluids, and washings of body cavities. Newer enzymes are considere… Show more

“…The molecule stimulates directly the ability of tumor cells to produce lung metastatic nodules [47,48]. Clinically AMF/PGI expression examined by IHC [33][34][35] as well as enzymatic activity in serum [49] has been shown to be associated well with metastatic progression in malignant tumors. It is interesting that hypoxia-induced metastasis is dependent on AMF/PHI activity in pancreatic carcinoma [46].…”

“…However, this test alone has certain limitations, since CEA may not be elevated in primary tumours, especially Dukes A and B lesions, or when there is a minimal residual tumour load remaining after resection of the primary tumour. The probable site of the metastases is not indicated by the level of CEA, although this omission can be overcome, at least in part, by the synchronous measurement of serum enzymes that reflect disturbances of hepatic function (Cooper et al, 1975;Munjal et al, 1976;Schwartz, 1976) but these tests do not become positive until there is a considerable tumour load, as judged by inspection of the liver at laparotomy (Cooper et al, 1976a). In a search for other biochemical tests that might be used to form an array for the monitoring of colorectal cancer, our attention has turned to the a globulins.…”

“…In addition to the serum PAP there are several other tests that have been used in an attempt to aid in the clinical evaluation and staging of patients with prostatic cancer. Serum alkaline phosphatase frequently is elevated markedly in patients with osteoblastic metastases [57]. The bone isoenzyme of serum alkaline phosphatase appears to be more specific, particularly in the presence of liver disease.…”
