Objective: To determine the current state of urgent neurologic in-hospital consultations, this study investigated urgent neurologic in-hospital consultations in a large tertiary hospital in China over a 4-year period. Methods: We conducted a retrospective study of urgent neurologic in-hospital consultations in a large tertiary hospital for 4 consecutive years (Jan. 1, 2014 to Dec. 31, 2017). The clinical data of patients who received urgent neurologic in-hospital consultations were obtained from the electronic medical record system and analyzed. Results: A total of 1437 cases (age range: 9-103 years old; average age: 60.3; males: 54.6%) were included in the study; 57.5% of the consultations were in line with urgent consultation criteria. The departments applying for urgent consultation involved 29Twenty-nine clinical departments were involved in the urgent consultations. consciousness disorder (36.0%), tics/stiffness (13.6%), limb weakness (8%), and mental disorders (5.6%). Urgent consultation diseases were divided into three categories: neurological diseases (77.8%), non-neurological non-neurological diseases (10.4%), and undiagnosed diseases (11.8%). The common neurological disorders included urgent cerebrovascular disease (33.6%), epilepsy/status epilepticus (15.8%), and metabolic or infective toxic encephalopathy (14.9%). Conclusions: Most urgent neurologic consultation is in line with consultations followed the guidelines for urgent neurologic consultation criteria. Non-neurologic. Non-neurological physicians in clinics should enrich their knowledge of frequently occurring urgent and critical neurological diseases.

Background Clinical neurology is difficult for young residents. To familiarize with neurological emergencies as soon as possible for young doctors, the urgent inpatient neurologic consultations were analyzed. Methods A retrospective study was conducted on the urgent inpatient neurologic consultations in a large tertiary hospital for 4 consecutive years. Results A total of 1437 cases were included, and the annual consultation cases gradually decreased from 573 to 257, involving 29 clinical departments. The disorders of urgent inpatient neurologic consultations were divided into three categories: neurological disorders (77.8%), non‐neurological disorders (10.4%), and undiagnosed disorders (11.8%), common causes in consultation were disturbance of consciousness (36.0%), convulsions/stiffness (13.6%), limb weakness (8%), and mental disorder (5.6%). Common neurological disorders included acute cerebrovascular disease (33.6%), epilepsy/status epilepticus (15.8%), and metabolic or infectious toxic encephalopathy (14.9%). Conclusion Urgent inpatient neurologic consultations involve multidisciplinary critical diseases, mainly neurological diseases. The standardized training of residents may help to rapidly improve the comprehensive diagnosis and treatment ability of young residents and is suitable for use in hospitals at all levels.

Objective. Neurologists and epileptologists are scarce in sub-Saharan Africa (SSA). Whilst electroencephalograms (EEGs) are becoming more available in the region, interpretation is typically undertaken by non-specialist clinicians with limited or no training. This is a systematic review of the peer-reviewed literature on EEG training of non-specialist clinicians worldwide, assessing the efficacy of the training methodology and the curricula content. Methods. The published literature was searched for papers relating to EEG training of non-specialist clinicians worldwide (1/01/1989-30/06/2020). All regions of the world were included and assessed for content on efficacy of curricula and potential adaptability or applicability to resource-poor settings. The grey literature was searched using ProQuest and Primo databases and references from review articles. The websites of the International League Against Epilepsy, International Federation of Clinical Neurophysiologist, American Academy of Neurology and World Federation of Neurology were reviewed for reports (non-peer reviewed) which described roll-out and impact of novel EEG training curricula. Results. There was limited data. From 2,613 articles, 15 complied with the study question. Ten studies were performed on cross-speciality clinicians, four on neurology registrars and one on a combination of healthcare workers. There was diversity of curricula models used. The studies themselves lacked consistency and directness. A few training programs were trialled in low-middle-income countries (LMICs) and paediatric training was included in only two. An ideal training curriculum was not evident nor evaluated for resource-poor settings. However, diverse teaching models were reported and set the foundations for further development of EEG training curricula for non-specialist clinicians. Significance. There is a lack of access to education in EEG training and interpretation for non-specialist clinicians in LMICs. Existing models need to be expanded or adapted and evaluated for this population group.
