Fluctuations of interleukin-1β (IL-1β) and heparin-binding epidermal growth factor (HB-EGF) in endometrial receptivity were detected. Seventy-two patients receiving in vitro fertilization-embryo transfer (IVF-ET) for the first time in Yantaishan Hospital from July 2015 to September 2015 due to infertility were selected. The serum and follicular fluid of patients in ovulation-promoting cycle were collected; the levels of IL-1β and HB-EGF in serum and follicular fluid were detected via enzyme-linked immunosorbent assay (ELISA), and the levels of serum estradiol (E2), progesterone (P), follicle stimulating hormone (FSH) and luteinizing hormone (LH) were detected. The endometria in early follicular phase and middle luteal phase were collected, and the mRNA expression levels of IL-1β and HB-EGF were evaluated by reverse transcription quantitative polymerase chain reaction (RT-qPCR). Patients were divided into two groups, and the implantation group (n=33) and the non-implantation group (n=39), according to whether embryos were implanted and the general data. In IVF-ET, the levels of IL-1β and HB-EGF in follicular fluid and middle luteal phase, the level of serum IL-1β on human chorionic gonadotropin (HCG) day and embryo transfer (ET) day, the levels of E2, FSH and LH on HCG day in implantation group were obviously higher than those in non-implantation group (p<0.05); the level of P on ET day in implantation group was significantly higher than that in non-implantation group (p<0.05); the expression levels of IL-1β and HB-EGF in endometrium in middle luteal phase in implantation group were higher than those in non-implantation group (p<0.05); the expression levels of IL-1β and HB-EGF in endometrium were positively correlated with the levels of E2 and P, and endometrial thickness (p<0.05). IL-1β and HB-EGF may improve the endometrial receptivity to embryo, thus affecting the embryo implantation rate, through the synergistic action with E2 and P, so they may be the indexes of predicting the IVF-ET pregnancy outcome.

Objective This study aimed to assess the effect of sildenafil citrate and estradiol valerate as adjuvant therapy during ovarian stimulation cycles with clomiphene citrate in patients with unexplained infertility to update infertility management guidelines in Kisangani. Method A double-blind, randomized controlled trial was conducted for two years (from October 1, 2021, to October 31, 2023). The population included 148 patients, 74 of whom were on clomiphene citrate + sildenafil citrate (CCSC) regimens and 74 of whom were on clomiphene citrate + estradiol valerate (CCEV) regimens for three months. The primary indicator was the conception rate, with secondary outcomes encompassing endometrial thickness, appearance and vascularity, the number of mature follicles and ovulation rate. Results Perifollicular flow was predominantly high-grade (P = 0.002), and the ovulation rate and clinical pregnancy rate were greater (P = 0.006) in the CCSC group than in the CCEV group. However, biochemical pregnancy rate, endometrial vascularity/thickness, and the number of Graafian follicles were not significantly different. The two groups were comparable in terms of sociodemographic and clinical characteristics. The mean duration of attempting to conceive was 4.39 years versus 4.36 years, and the mean AFC was 11.51 versus 11.46, respectively, in the CCSC versus CCEV groups. Secondary infertility was the most frequent type in each of the two participant assignment groups. Conclusion As an adjuvant, sildenafil increases the rate of clinical pregnancy more than does estradiol in patients with unexplained infertility undergoing ovarian stimulation with clomiphene citrate. However, both sildenafil citrate and estradiol valerate can protect the endometrium from the harmful antiestrogenic effects of clomiphene. AbstractThe mechanisms involving the expression of interleukin (IL) 1 family members in the process of preparing the endometrium to receive an embryo remain unclear. In this study, decidualization differentially skewed the balance of IL1 family receptor expression in a pattern that increases endometrial stromal cell receptivity to IL1, IL18 and IL33. Additionally, endometrial cells showed increased expression of homeobox HOXA10 and HOXA11 and LIFR, which are known to be involved in endometrial embryo receptivity. Further analyses of decidual endometrial cells revealed a significant increase in the release of potent proinflammatory, remodelling and angiogenic factors implicated in the embryo invasion process, such as VEGF (P = 0.0305), MMP9 (P = 0.0003), TIMP3 (P = 0.0001), RANTES (P = 0.0020), MCP1 (P = 0.0001) and MIF (P = 0.0068). No significant changes in endogenous IL1B secretion were observed. Decreased secretion of IL18 and decidualization increased secretion of IL33. These findings reveal a significant modulation of endometrial cell receptivity to IL1 family members during endometrial stromal cell decidualization, and suggest that the involvement of IL1 family members is important in physiological processes of endometrial receptivity, including adaptive immunology. This may be relevant to establishing a favourable uterine microenvironment for embryo implantation.
