Kytococcus schroeteri Pneumonia in Two Patients with a Hematological Malignancy

Abstract: Neutropenic patients are susceptible to infections with usually harmless microorganisms. We report two cases of severe pneumonia in hematological patients due to Kytococcus schroeteri, a saprophyte of the human skin. When blood cultures or respiratory specimens yield micrococcus-like colonies, Kytococcus species, which are often resistant to penicillin, should be considered and the antimicrobial therapy should be adjusted accordingly.

“…Our literature review also identified 6 reported cases of Kytococcus pneumonia. Five of these were in conjunction with hematological malignancy (4 AML and 1 hairy cell leukemia) [1113, 17]. The sixth patient had no hematologic condition but was moderately immunocompromised by taking prednisone 20 mg daily for 2 years for management of refractory asthma [3].…”

“…K. schroeteri is an underrecognized and underreported organism. Hodiamont et al reported on two cases of pneumonia and bacteremia caused by K. schroeteri [ 4 ]. In their research they came across six cases of pneumonia and bacteremia that were previously identified as Micrococcus sp.…”

“…However, K. schroeteri can also cause severe to fatal infections. So far, K. schroeteri has been identified as the causative agent in cases of prosthetic valve endocarditis (n55) (Aepinus et al, 2008;Becker et al, 2003;Mnif et al, 2006;Poyet et al, 2010;Renvoise et al, 2008), pneumonia (n53) (Hodiamont et al, 2010;Mohammedi et al, 2005), spondylodiscitis (n51) (Jacquier et al, 2010) and VPS infection (n51) (Jourdain et al, 2009).…”

“…Kytococci seem to be intrinsically resistant to penicillin and oxacillin (Becker et al, 2002). Therefore, the antimicrobial therapy reported in other case studies included vancomycin and gentamicin (Hodiamont et al, 2010) plus rifampicin (Becker et al, 2003;Aepinus et al, 2008;Mnif et al, 2006), or vancomycin plus gentamicin (Jourdain et al, …”
