Human Immunodeficiency Virus (HIV) is a retrovirus originating from the retroviridae family of the genus lentivirus that infects and damages cells that have a molecule Cluster of Differentiation 4 (CD4), especially T lymphocytes that have receptors with high affinity for HIV. Total lymphocyte count / TLC has been proposed as an alternative guide to CD4 in limited health facilities. This study aims to determine the correlation between Total Lymphocyte Count (TLC) and CD4 in HIV/AIDS patients in the W.Z. Johannes Kupang hospital. The type of this research was observational analytic with a cross-sectional design. The study was conducted on 121 samples of patients who performed CD4 examination and Total Lymphocytic Count (TLC) in the laboratory of W.Z.Johannes Kupang Hospital. The Spearman correlation test shows a significance value of 0,000 with a Spearman correlation value of 0.799. Based on the results of this study it can be concluded that there is a significant correlation between Total Lymphocyte Count and CD4 and shows the direction of positive correlation with a very strong relationship, where the increase in the number of Total Lymphocyte Count is in line with the increase in CD4 counts.

Introduction: The Province of West Java is one of the highest HIV sufferers in Indonesia that has increased accumulatively in 2013 to 2014. This condition was proven that in 2014 West Java ranked 4th among the provinces with the highest HIV sufferers in Indonesia. HIV has main target to infect a cell that contains of receptor, CD4. CD4 examination routinely is very important to decide HIV replication process. Meanwhile, CD4 test in the laboratory is quite expensive and not always available in every health facilities. Mangosteen peels extract contains of xanthones as an antioxidant, which is needed for body as a prooxidant that can inhibit the replication of HIV and the activity was seen from the total number of lymphocyte. Method: The human experimental research has been done by Randomized Control Group Pretest-Post Test Design. There were 20 patients sample for each group. In group 1, HIV patients with ARV therapy were given mangosteen peels extract and in group 2, they were given placebo capsule. Wilcoxon Smith test and Mann Whitney U test were used to determine the difference between group 1 and group 2. Result: There is no significant difference between lymphocytes (p: 0.102) to HIV patients with ARV in each group examination. Discussion: The ineffectiveness is caused by the phagocytocity and cytoxity of TNF through the increasing of free radicals in HIV patients.

ABSTRAK Wanita hamil rentan terhadap penyakit infeksi salah satunya adalah Covid-19. Covid-19 memicu terjadinya hiperinflamasi yang menyebabkan kerusakan jaringan paru dan disfungsi organ. Total Leukocyte Counnt (TLC) dan Neutrophil Lymphocyte Ratio (NLR) merupakan indikator untuk memprediksi tahap awal dan potensi perkembangan penyakit yang cukup mudah dilakukan utamanya di fasilitas kesehatan yang terbatas. Tujuan dari penelitian ini adalah untuk mengetahui perbandingan nilai TLC dan NLR pada pasien Covid-19 wanita hamil dan tidak hamil. Hasil penelitian menunjukkan bahwa pasien Covid-19 kelompok hamil dan tidak hamil sama-sama berada pada standar normal walaupun memiliki kecenderungan jumlah neutrofil yang tinggi dan jumlah limfosit yang rendah yang umum terjadi pada pasien Covid-19. Nilai TLC dan NLR pada pasien Covid Wanita hamil dan tidak hamil tidak berbeda signifikan (p>0,05). Persentase jumlah pasien dengan nilai NLR pasien Covid-19 hamil dan tidak hamil <5.8 yaitu 70% dan 67%, sedangkan di atas >5.8 yaitu 30% dan 33%. Hal ini selaras dengan hasil bahwa persentase pasien bergejala ringan lebih banyak dibandingkan dengan pasien bergejala sedang dan berat. Kondisi kehamilan yang rentan terhadap penyakit infeksi dengan sistem imunitas yang unik membuktikan kondisi ini mampu menurunkan potensi perkembangan Covid-19 yang lebih serius.
