Objective:The aim of the present study is to evaluate whether a "radiation free" method using 3D facial scan can replace Cone Beam Computed Tomography (CBCT) volumetric rendering of soft tissue of the patient to assess maxillofacial surgery outcomes and compare the reference points and angular measurements of patient facial soft tissue. Material and Methods: Facial soft tissue scan of the patient's face, before and after orthognathic surgery and a CBCT of the skull for volumetric rendering of soft tissues were carried out. The 3D acquisitions were processed using Planmeca ProMax 3D ProFace® software (Planmeca USA, Inc.; Roselle, Illinois, USA). The participant were positioned in a natural position during the skull scannering. Three sagittal angular measurements were performed (Tr-NA, Tr-N-Pg, Ss-N-Pg) and two verticals (Go-N-Me, Tr-Or-Pg) on facial soft tissue scan and on the patient's 3D soft tissue CBCT volumetric rendering. Results: A certain correspondence has been demonstrated between the measurements obtained on the Proface and those on the CBCT. Conclusion: A radiation free method was to be considered an important diagnostic tool that works in conditions of not subjecting the patient to harmful ionizing radiation and it was therefore particularly suitable for growing subjects. The soft tissue analysis based on the realistic facial scan has shown sufficient reliability and reproducibility even if further studies are needed to confirm the research result.

Background Anatomical analysis of the sphenoid sinus is important because of its proximity to vital neurovascular structures. The aim of this study is investigating the anatomy of the SS and its surrounding structures in CBCT images. Methodology: In this retrospective cross-sectional study, 201 cone-beam computed tomography (CBCT) radiographs were analyzed. The analyzed images included the type of SS, protrusion of ICA and optic nerve and foramen rotundum, vidian canal type, the Onodi cell, pneumatization to surrounding bones, the ostium, the septum. The data were analyzed using chi-square and the t-test in SPSS 24 at a significance level of 0.05. Results The tilted septum had the highest frequency (46.3%). Ostium was bilateral in most images (56.2%). The protrusion of the ICA (54.7%) and optic nerve (62.2%) was smooth in most images. Foramen rotundum protrusion and Onodi cell prevalence were 25% and 22.9%, respectively. The postsellar type of sinus had the highest frequency (71.1%). Vidian canal Grade II had the highest frequency (40.3%). Greater wing pneumatization had the highest frequency (51.7%). No statistically significant relationship was found between the optic nerve protrusion and Onodi cell (P-value < 0.05). There was a significant relationship between the pneumatization of the pterygoid process and the protrusion of the vidian canal, and the foramen rotundum protrusion (p-value < 0.001). Conclusion CBCT imaging is very effective in the anatomical analysis of the SS and its surrounding structures. The postsellar type had the highest frequency.Vidian canal Grade II had the highest frequency; greater wing pneumatization was observed in more than half of the images; the most common type of septum was tilted; ostium was bilateral in most cases; the protrusion of the ICA and optic nerve was mostly smooth.

Objectives This study aimed to assess the accuracy of different fields of view (FOVs) and voxel sizes of cone-beam computed tomography (CBCT) for detection of simulated periodontal defects in comparison with periapical (PA) radiography. Materials and Methods This in vitro study was conducted on 60 periodontal defects including furcation and wall defects, dehiscence, and fenestration that were artificially created in 5 mandibles of freshly slaughtered sheep. After recording the defect sites and types as the gold-standard, they underwent PA radiography. Thirty CBCT images were also obtained in three FOVs and voxel sizes of 13 x 17 cm2/ 300 µm, 10 x 10 cm2/ 180 µm, and 5 x 5 cm2/ 90 µm, and evaluated by a radiologist and a periodontist. The inter-observer agreement was assessed by the kappa coefficient, and the parameters were calculated and compared using the Chi-square test with STATA 14. Results The inter-observer agreement was generally 85.4% for all periodontal defects; this value was the highest (88.1%) with the smallest FOV and voxel size (5 x 5 cm2 and 90 µm). By a reduction in FOV and voxel size, the inter-observer agreement significantly increased (P < 0.001). The mean sensitivity, specificity, PPV, and NPV were 87.8%, 94.9%, 94.6%, and 88.4%, respectively, for CBCT. These values for PA radiography were lower than the corresponding values for CBCT. Conclusion Sensitivity, specificity, PPV, NPV, and inter-observer agreement increased in use of smaller FOV for detection of simulated periodontal defects, indicating an increase in diagnostic accuracy by a reduction in FOV and voxel size.
