Kimura’s disease mimicking an earlobe keloid

Abstract: Kimura's disease (KD) is a rare chronic inflammatory disorder of unknown aetiology. It usually presents as soft-tissue masses predominantly in the head or neck region. We report a case of an asymptomatic tumour on the left earlobe mimicking a keloid. The tumour was histopathologically diagnosed as KD. This case suggests the importance of conducting skin biopsies of keloidal lesions on head and neck regions, particularly in patients who have peripheral eosinophilia and increased IgE levels.

“…Hence, we proposed to call these newly discovered, easily missed LAs ‘keloid‐associated lymphoid tissue’ (KALT). The presence of LAs has previously been reported in other scarring conditions, such as burn scars 68 and Kimura’s disease 69 . In the present study, all the LAs seen expressed VCAM‐1 although some were CD21–, suggesting that these cases represent a relatively early stage of KALT 62 .…”

“…The presence of LAs has previously been reported in other scarring conditions, such as burn scars 68 and Kimura's disease. 69 In the present study, all the LAs seen expressed VCAM-1 although some were CD21-, suggesting that these cases represent a relatively early stage of KALT. 62 The low level of Ki67 immunoreactivity of KALT suggests that these KD-associated LAs are much less proliferative compared with lymph nodes and other secondary lymphoid tissue, as already reported in idiopathic pulmonary fibrosis.…”

“…This is particularly important because various malignant and benign tumors can mimic keloid scar [5,6]. This is exemplified in our case in which the initial biopsy revealed it as DF.…”

“…Among the malignant tumors that mimic keloid scars are dermatofibrosarcoma protuberans, trichilemmal carcinoma and keloidal basal cell carcinoma [5]. In addition to malignant skin conditions, there are a few benign conditions that also clinically resemble keloid scar [5,6]. These included nodular scleroderma, keloidal scleroderma, sclerotic neurofibroma, keloidal granuloma, erythema elevatum diutinum, dermatofibroma, penile edema, pseudoangiomatous hyperplasia, lichen sclerosus, apocrine cystadenoma, adult-onset juvenile xanthogranuloma, chondroid syringoma and subcutaneous eosinophilic lymphoid granuloma [5].…”
