Keloids Treated with Topical Injections of Triamcinolone Acetonide (Kenalog): Immediate and Long-term Results

Abstract: In a prospective trial, the results of local steroid treatment of keloids have been studied. 52 patients were treated with Kenalog (triamcinolone acetonide) injections alone, whilst in 15 patients, steroid therapy was combined with excision of the keloid. The combined treatment gave no better results than injection therapy alone. The early result of treatment was the complete flattening of the lesions and cessation of itching in the majority of cases. However in one-third of the cases partial recurrence was fo… Show more

“…Overall, this modality has a high degree of tolerability as well as effectiveness in reducing symptoms. Several studies evaluating intralesional steroids have reported roughly a 50% recurrence rate [2,3]. In the recent past, intralesional 5-FU has been tried in hypertrophic scars and keloids in combination or as an individual therapeutic agent [1,[4][5][6].…”

“…A paucity of literature is available regarding the usefulness and safety profile of 5-FU as an individual therapeutic agent [1]. However, several studies suggest that the overall efficacy is not better than other modalities and significant side effects such as ulceration and hyperpigmentation make topical 5-FU less appealing [1][2][3][4][5]. Hence, we conducted a prospective trial to compare the efficacy of 5-FU versus triamcinolone as a therapeutic agent for the treatment of keloids.…”

“…12 The main effect of intralesional steroids on the connective tissue of keloids is a decrease in viscosity due to a loss of ground substance. 14 Few complications have been documented, and are limited to local skin changes or pain as a result of the injection.…”

“…As early as in the 1960s, authors reported curative results in up to 80% of the cases [9][10][11][12]. Other, more recent studies suggest recurrence rates between 9% and 50% with this line of treatment [13][14][15][16][17]. Alternative treatment options include surgery followed by radiotherapy, and other injectable substances, such as 5-fluorouracil, bleomycin and verapamil [18].…”
