Kaposi Sarcoma After Bilateral Lung Transplantation

Abstract: We describe radiologic findings in 2 patients and 18-fluoro-deoxy-glucose positron emission tomography/computed tomography findings in 1 patient who developed Kaposi sarcoma of the lung after bilateral lung transplantation. These findings included lung nodules, mediastinal or hilar lymphadenopathy, and pleural fluid.

“…In the ISHLT registry, at least one malignancy was reported in 13% of lung transplant recipients who survived five yr and in 28.2% who survived 10 yr. The most common malignancies were lymphoma and skin cancer, but other types of malignancies such as Kaposi sarcoma, breast, lung, and colon carcinoma among others have been reported after SOT . Although the higher cancer rate is thought to be due to the immunosuppressive therapy given to recipients and to the baseline risk factor of the patient , radiation exposure may also contribute an additional cancer risk.…”

“…To the best of our knowledge, there are a few cases of Kaposi sarcoma with only graft involvement and only 1 previous report with lung allograft involvement without any related symptoms. [8][9][10] The succession of this unfortunate case underlines the potential morbidity of transplant-related Kaposi sarcoma and the need to maintain a high level of suspicion for Kaposi sarcoma in this population, even in the absence of cutaneous involvement and significant initial respiratory distress.…”

“…One of the significant advantages of FDG PET/CT is its routine whole-body acquisition, which is ideal for systemic disease such as KS. In the radiology literature, there were a few case reports about applications of FDG PET/CT for AIDS-defining KS, which all suggested that FDG PET-CT was effective in detecting clinically occult KS lesions that were difficult to diagnose with traditional imaging techniques in more advanced stages of KS [14][15][16][17][18].…”
