Judicious use of oxytocin augmentation for the management of prolonged labor

Abstract: The frequency of emergency cesarean section was reduced after implementing judicious use of oxytocin augmentation. Our findings may be of interest in the ongoing discussion of how the balanced use of oxytocin for labor augmentation can best be achieved.

“… 27 The role of oxytocin in labour management is important, but the optimal dose and timing is yet to be revealed. 26 The different types of labour management probably explain the different rates of oxytocin augmentation and prolonged labours observed ( tables 3 and 4 ).…”

“…SUH practice a judicious use of oxytocin that includes a definition of the start of active labour, prolonged labour and thereby indication for oxytocin use, which differ from NMH and SLO. 26 Compared with NMH and their philosophy of prevention of prolonged labour this may lead to more labours that are prolonged. The package of Active Management of Labor with one to one care and its advantages practised at NMH and in SLO has always been an issue of much debate.…”

“…Prolonged labor, also known as failure to progress, was considered when the labor, after the latent phase of first stage of labor, exceeds 12 hours in primigravida or 8 hours in multipara mothers. [22][23][24] Premature rupture of membrane (PROM) was considered as a rupture of the membrane of the amniotic sac and chorion occurred more than one hour before the onset of labor. Prolonged rupture of membrane is a rupture of membrane or leaking for >18 hours.…”

“…Similar to induction, the reason for slow progress may become the reason for emergency CS. As oxytocin is a powerful drug with potential serious side-effects, minimizing its use is generally advised 32 . The rate of augmentation, excluding PROM, induction, multiple pregnancy and elective CS, was 16.6% (741/4481) for all, 10% for multiparous, and 29.9% for primiparous women.…”
