iStent as a Solo Procedure for Glaucoma Patients: A Systematic Review and Meta-Analysis

Abstract: BackgroundGlaucoma is a leading cause of irreversible blindness. It is firmly entrenched in the traditional treatment paradigm to start with pharmacotherapy. However, pharmacotherapy is not benign and has been well documented to have a number of significant challenges. Minimally invasive glaucoma surgery (MIGS) that targets the outflow pathway with minimal to no scleral dissection has resulted in the need to reconsider the glaucoma treatment paradigm.PurposeTo perform a systematic review and meta-analysis to e… Show more

“…The meta‐analysis also demonstrated a significant reduction in the number of glaucoma medications used after the procedure in the combined group compared with the phacoemulsification only group (weighted mean reduction of 1.33 from baseline compared with 1.01) . Significant reduction in IOP from baseline has also been demonstrated in patients with mild to moderate glaucoma treated with stent insertion alone at follow up of 6 to 18 months …”

“…4 Significant reduction in IOP from baseline has also been demonstrated in patients with mild to moderate glaucoma treated with stent insertion alone at follow up of 6 to 18 months. 5 Although each trabecular micro-bypass device has proven to be efficacious, the two devices have not been previously compared in performance and efficacy.…”

“…We observed a greater decrease in IOP from a higher baseline IOP compared with a prior study that uses a trabecular microbypass stent . In that study, the IOP decreased with standalone iStent implantation 22% from a baseline near 24 mmHg on one less medication ( vs . our AIT 47% decrease from a baseline of 32 mmHg on 1.5 less medications).…”
