The result of surgical correction of hydronephrosis caused by chronic obstruction at the pyeloureteral junction has been evaluated clinically as well as by urography and body background substracted renography performed preoperatively and up to 65 months postoperatively. The average follow-up period was 24 months. Twenty-eight patients (29 kidneys) were operated. Clinically 24 patients were improved. Urography showed regress of hydronephrosis and/or improved urinary pelvic drainage in 22 patients, whereas kidney function is judged from renography was improved in only 8 patients. Improvement in all parameters was generally most marked during the first postoperative months. Renographic improvement was seen much more often when the contralateral (non-operated) kidney was abnormal. Thus, kidney function in terms of effective renal plasma flow is rather uninfluenced by hydronephrosis surgery if the contralateral kidney is normal, whereas clinical improvement in urinary drainage is regularly obtained. This improvement in drainage is poorly registered by renography. It is concluded that renography is an important complementary method to urography, but the method cannot replace the radiological methods in the follow-up of hydronephrosis surgery.

A comparison was made between the excretion urogram and the radio-isotope renogram in 35 patients with uretero-sigmoid anastomosis. A good correlation was obtained between the 2 tests in the presence of radiographic evidence of (a) hydronephrosis, (b) poor renal function and (c) absent function. Renographic evidence of mild impairment of secretory function and of delayed excretion was associated with a significant number of normal excretion urograms and in a few instances there was later progression to severe renal impairment or hydronephrosis. Hydro-ureter associated with a normal renogram carried a good functional prognosis. Renography is recommended in the long-term supervision of patients with uretero-sigmoid anastomosis.

The results of excretion urography (IVU) and dynamic renal scintigraphy using 99mTc DTPA(Sn) were compared in 109 urological patients. Computer analysis of the gamma camera data provided a reproducible, accurate, qualitative and quantitative assessment of renal function and was found to be superior to urography in the detection of morphological defects of the renal parenchyma and in the identification of early upper tract obstruction. The radio-isotope study is a safe, non-invasive investigation, easily performed on an out-patient basis, requires no prior preparation and is without significant radiation hazard to the patient.
