Isolated Noncompaction of the Left Ventricular Myocardium in an Elderly Patient

Abstract: Noncompaction of the ventricular myocardium (NVM) is a rare disorder of endomyocardial morphogenesis characterized by numerous, prominent trabeculations and deep intertrabecular recesses. It is commonly associated with congenital heart disease, but the isolated form (INVM) is not associated with other structural heart diseases. Clinical reports of INVM have been limited to a few case reports and small series of pediatric patients. INVM is considered to be a form of congenital abnormal endomyocardial morphogene… Show more

“…Histological work-up of LVHT in an 80-year-old female with metabolic myopathy revealed atheromatous arteries within the trabeculations, some necrotic cardiomyocytes, fibrocytes, hit cells and scars [9]. Fibrosis within the trabeculations is attributed to ischemia by some authors [10]. In the present patient, trabeculations were built up of a regular myocardial texture without an increase in fibrosis or fat tissue.…”

“…1 However, we want to mention the following concerns. The authors say that LVHT is exclusively a congenital disorder and do not seem aware that indivdual patients may acquire LVHT during their lifetime.…”

“…1 According to previous reports, isolated noncompaction of the left ventricular myocardium (INVM) results from a congenital disorder in which abnormal endomyocardial morphogenesis is caused by abnormal cessation of embryonic development of the left ventricular myocardium. The nomenclature of noncompaction is thus derived from this disorder of embryonic development.…”
