Fetal hepatic calcifications can be caused by infections, chromosomal disorders, thrombotic events, ischemic hepatic necrosis and subcapsular hematomas among others events. Its features and clinical significance are still not well known. We performed an observational study to describe fetal hepatic calcifications and its association with main clinical and histopathological findings from the fetal autopsy database, between 2007 and 2014. Raw odds ratio analysis was performed. We reviewed 591 fetal autopsies: 14 cases with hepatic calcifications, 102 fetuses with chromosomal disorders; 13 with diagnosis of TORSCH (toxoplasma, rubella, syphilis, cytomegalovirus, herpes virus 1 and 2, and others) and 207 with any abnormality in the umbilical cord (UC). The relation between hepatic calcifications and chromosomal disorders in our series had significance. It is known that hepatic calcifications are common in chromosomal disorders, transplacental infections and UC abnormalities, those conditions are risk factor for hepatic calcifications formation; we suggest hepatic calcifications should alert the pathologists in order to consider these etiologies in first instance.

Congenital liver calcification. Shall we worry or not? That is the questionLas calcificaciones hepáticas fetales son un hallazgo relativamente frecuente. Pueden ser superficiales o intrahepáticas. Su presencia se ha relacionado con múltiples patologías (peritonitis meconial, infecciones connatales, cromosomopatías, isquemia, tumores, etc.). A pesar de su alta frecuencia, su manejo no está debidamente protocolizado.Presentamos un caso de calcificación hepática diagnosticada en la semana 24 de embarazo, confirmada posnatalmente y de evolución favorable.Fetal liver calcifications are a relatively common finding. They may be superficial or intrahepatic. Their presence has been linked to many diseases (meconium peritonitis, connatal infections, chromosomal abnormalities, ischemia, tumors, etc.). With advancing technology, it is becoming common for antenatal ultrasound to detect echogenic lesions in fetal abdomen. Paucity of data in this field, however, makes it difficult to counsel patients. We report four cases of fetal liver echogenic lesions, postnatal outcome (delivered during 2015-2016) and a literature review to increase awareness. Intrahepatic calcification is relatively common with an incidence of approximately 5-10 in 10,000 pregnancies. Prenatal detection of echogenic lesions in fetal abdomen causes huge anxiety and stress to parents; therefore, it is important for the ultrasonographers to be up to date with the evidence-based management of these lesions. Most lesions would carry no or little risk to neonate; however, few cases may require careful planning to optimise the time and place of delivery. We describe four cases between February 2015 and December 2016 using machine Voluson S6 and E8.
