Isolated Capsulorhexis Flap Technique in Femtosecond Laser-Assisted Cataract Surgery to Protect the Corneal Endothelial Cells

Abstract: Isolated capsulorhexis flap technique initiate a new approach to protect the endothelial cell. It is a simple technique with a short learning curve, but still need a further investigation to prove the protection of corneal endothelial cells.

“…Over the years, different methods have been proposed to make cataract surgery easier and safer, but the revolutionary breakthrough in this area occurred with the development of phacoemulsification. With the use of this technique, it has been possible to achieve a good standard of low-risk efficacy compared to the techniques previously used [9]. The advent of Femto Laser-Assisted Cataract Surgery (FLACS) has indeed decreased the use of ultrasonic energy in the anterior chamber.…”

“…Для защиты эндотелиальных клеток, особенно при низком их количестве, стандартом на сегодняшний день является использование вискохирургической техники «мягкой оболочки» (soft-shell technique) по S. Arshinoff [13]. Еще одним способом защитить эндотелий при проведении факоэмульсификации катаракты является предложенный S. Li et al (2017) так называемый «слоеный пирог» [14].…”

“…[16][17][18] Adverse effects at cellular and subcellular level that can contribute to ocular inflammation during cataract surgery are attributed to mechanical, thermal, and chemical mechanisms. Surgical instruments contact and turbulent fluids which are generated by the phaco tip's jackhammer effect are the causes of mechanical injury, 19,20 while high frequency ultrasound vibration at the tip or occlusion of the tip caused by the lens fragmentation during the emulsification are the causes of the thermal injury. 20,21 The imploding of cavitation bubbles that are generated during the procedure causes a chemical damage that is mediated by free radicals, formed in the aqueous humor.…”
