Хирургия катаракты является одним из самых динамично развивающихся направлений в офтальмологии. Стандартом современной хирургии катаракты на сегодняшний день остается метод ультразвуковой факоэмульсификации (ФЭК). Достаточно часто оперативное вмешательство может быть осложнено рядом неблагоприятных факторов: слабостью цинновых связок, высокой плотностью ядра, недостаточностью эндотелиального слоя клеток роговицы, узким ригидным зрачком, аниридией. Современная оптимизация всевозможных процессов привела к тому, что при проведении ФЭК активно используется фемтосекундный лазер. Наряду с успешным выполнением собственно ФЭК важнейшим аспектом работы катарактального хирурга является правильный выбор интраокулярной линзы (ИОЛ). Разработка асферичных ИОЛ позволила существенно улучшить функциональные результаты вмешательства, обеспечивая пациентам более высокие зрительные функции, в первую очередь в мезопических и никтопических условиях. В XXI в. появились новые возможности для пациентов, в частности полный отказ от очковой коррекции в послеоперационном периоде после хирургии катаракты с имплантацией мультифокальных ИОЛ. Одной из последних разработок является линза, позволяющая получить большую глубину фокуса с усилением псевдоаккомодации и использованием динамики зрачка. Внедрение таких линз может потребовать изменения представления о параметрах оптической системы глаза, что приведет к созданию новых формул расчета или модификации имеющихся и, возможно, обязательному учету хирургического вклада в рефракционный исход, а следовательно, откроет новый этап в развитии хирургии катаракты.
fractions of polypeptides (Retinalamin) were performed in the study group. Conventional treatment for glaucomatous optic neuropathy (antioxidants, vasodilators, nootropic drugs, and retinoprotectors) was performed in the control group. Results: in the study group, FAZ area reduced by 33% immediately after the treatment, by 28.7% after 1 month, and by 16.8% after 6 months compared to baseline. In addition, FAZ perimeter reduced by 26.7%, 17.6%, and 8.1%, respectively, compared to baseline. No significant changes in visual acuity, visual field indices, and FAZ parameters were revealed. Moderate and strong correlation between the mean deviation (MD) and FAZ area was revealed in both groups irrespective of treatment type (rstudy=-0.6044, р<0.05; rcontrol=-0.8249, р<0.05). Conclusion: OCTA assesses central retinal microcirculation by FAZ parameters. The changes in FAZ parameters illustrate treatment efficacy and correlate with functional improvement (as demonstrated by visual field testing). Hence, it is recommended to monitor FAZ parameters when providing neuroretinoprotective therapy to evaluate its effectiveness. Intravitreal injections (IVI) have revolutionized management strategies for many disorders, e.g., age-related macular degeneration (AMD), diabetic macular edema, retinal vein occlusion, choroidal inflammatory diseases, and pathologic myopia. Over the last two decades, exponential growth in the rate of IVIs has been seen worldwide. Primary therapeutic modalities include the courses of periodic injections to achieve and maintain the desired effect. Studies on the effects and neuroprotective role of vascular endothelial growth factor (VEGF) and agents blocking its action in various organs and tissues on the retina have demonstrated some differences in the outcomes and their interpretation which potentially affect IVI management strategy and treatment efficacy. Understanding the results and ocular changes in response to therapy (particularly in patients with glaucoma) requires special attention. A predicted increase in life expectancy and the number of patients with glaucoma suggests that the number of patients with several eye diseases who require IVIs will inevitably increase. In addition, some studies evaluate glaucoma progression after IVIs. Considering a lack of relevant data on long-term adverse effects on IOP level and cup area, and improvement of visual functions, IVIs are regarded as a safe treatment modality in patients with primary open-angle glaucoma (POAG) and ocular comorbidities that require IVIs.
