In the past several decades, increasing evidence supports the efficacy of psychotherapies for depression. The vast majority of findings from meta-analyses, randomized clinical trials (RCTs) and naturalistic studies have demonstrated that well-established psychotherapies (behavioural activation, problem-solving therapy, psychodynamic therapy, cognitive-behavioural therapy, interpersonal therapy and emotion-focused therapy) are superior to no-treatment and control conditions, and are in most cases equally effective in treating depression. However, despite this abundant support for psychotherapies, studies have also consistently shown high drop-out rates, high percentages of non-respondent patients who experience treatment failures, and mixed findings regarding the enduring effects of psychotherapy. Thus, there is a need to develop more personalised treatment models tailored to patients' needs. A new integrative sequential stepwise approach to the treatment of depression is suggested. What do we know about the efficacy of psychotherapies for depression?Depression is a pervasive mental condition which interferes with all domains of one's functioning and it is one of the most common problems leading people to seek treatment (e.g. Summers & Barber, 2010). Recent studies have shown that the approximated cumulative incidence of depression diagnosis from childhood to adulthood is between 32.54 and 51% (Rohde et al. 2012;Angst et al. 2015).In the past several decades, over 400 randomised clinical trials have investigated the efficacy of psychotherapy for depression (Cuijpers, 2015). Despite the heated debate regarding the superiority of one treatment over others, there is fairly wide acceptance of the 'Dodo Bird Verdict ' (Luborsky et al. 1975), at least in the case of depression, suggesting that all treatments are more or less equally effective (Cuijpers et al. 2008;Barth et al. 2013). Furthermore, research in the past two decades has taught us that psychotherapeutic treatments of depression are superior to waitlist and other control conditions (Barth et al. 2013;Lambert, 2013), and that psychotherapy combined with anti-depressant medication is more effective than medication alone (Barber et al. 2013;Köhler et al. 2013;Hollon et al. 2014;Fonagy, 2015;Leichsenring et al. 2015). There is also evidence suggesting that some types of psychotherapies are not only as effective as medication, but also have higher long-term enduring effects (Hollon & Ponniah, 2010;Cuijpers et al. 2013b). We begin by briefly reviewing some of the major findings that have arisen from the literature on treatments of depression, focusing only on the best-known and more widely researched. Se trata de un personaje de Alicia en el País de la Maravillas que resuelve una carrera con un planteamiento alocado (cada uno empezaba por donde y cuando quería, y paraba a su antojo) con la sentencia "Todos han ganado, y todos deben tener premio". En Psicología, el veredicto del pájaro Dodo ha valido para representar metafóricamente la idea de que todas las psicoterapias obtienen resultados equivalentes. De aquí se ha postulado que lo que prevalece en la eficacia de los tratamientos son los factores comunes referidos al paciente, al terapeuta, a la relación entre ambos, a la estructura de la terapia o al proceso terapéutico en sí, más que los componentes específicos de cada orientación terapéutica (Wampold, 2007).El presente artículo pretende repasarla historia de este debate, resaltar algunas de las limitaciones que deben contener las conclusiones que de él se pueden obtener y, por último, argumentar sobre los motivos por los que el debate sigue, como en el cuento, sin un claro final. LA HISTORIA DEL VEREDICTO DE DODOEl primer referente de este veredicto de Dodo se encuentra en 1936 un artículo de Rosenzweig (1936), que contiene la primera formulación de la preponderancia de los factores comunes en psicoterapia. Unas cuantas décadas después, la idea fue rescatada en una revisión cualitativa de la literatura que concluye que no existen efectos diferenciales entre los tratamientos (Luborsky, Singer y Luborsky, 1975). Smith y Glass (1977) fueron los primeros en hacer la primera revisión cuantitativa (meta-análisis) tratando de resumir los datos de 375 estudios. Concluyeron, por una parte, que todas las psicoterapias eran más eficaces que el no-tratamiento; por otra, que las diferencias entre las escuelas eran en la práctica despreciables, por lo que, en definitiva, no estaba justificada la reivindicación de que unas eran superiores a otras. Aunque el tamaño del efecto entre terapias es grande para unas (0.91 para la desensibilización sistemática) y pequeño-medio en otras (0.26 para la Gestalt), la conclusión se sostiene en la agrupación de las terapias entre conductuales y no-conductuales (entre las que incluye, junto a la rogeriana y las psicodinámicas, algunas cognitivas). Research over the last four decades on cognitive behavioral therapy (CBT) is leading to a more comprehensive understanding of the mechanisms that underpin its efficacy. This introduction to the next generation of research on process offers a brief account of the evolution in the scientific foundations of CBT, to our current focus on treatment and in-session processes. It also provides a generic model for linking techniques with their target, uses, and treatment processes. In addition, how each component can be adapted according to CBT-specific elements of the client-therapist relationship (collaboration, empiricism, and Socratic dialogue) in a manner that is guided by the case conceptualization provides a more complex and comprehensive understanding of treatment delivery. The various research studies included within this special issue make important contributions to our understanding of the different ways in which both treatment and in-session processes are important to CBT.
