BACKGROUND: Although medical financial hardship (MFH) resulting from sequelae of cancer and treatment has been reported in other racial/ethnic populations, little is known about MFH among Native Hawaiian and Pacific Islander (NHPI) cancer survivors. METHODS: One hundred fifty adult NHPI cancer survivors were identified from the 2014 NHPI National Health Interview Survey (NHIS). Cancer survivors were those with a history of cancer (excluding nonmelanoma/unknown type of skin cancer). MFH was defined by 3 domains: 1) material (problem paying or unable to pay medical bills); 2) psychological (worrying about paying medical bills); and 3) behavioral (delaying or forgoing medical care for financial reasons). For comparison, 2098 non-Hispanic white (NHW) cancer survivors were identified from the 2014 NHIS. Logistic regressions were used to identify sociodemographic and health factors associated with experiencing MFH. Stratified analysis by age (<65 vs ≥65 years) and sensitivity analysis using propensity score-matched NHPI and NHW cancer survivors were conducted. Nationally representative estimates were generated using survey weights. RESULTS: Among elderly cancer survivors, more NHPIs reported any MFH (59% vs 32%; P = .001), psychological MFH (36% vs 22%; P = .040), and behavioral MFH (27% vs 9%; P = .004) than NHWs. Among survivors aged <65 years, NHPIs reported less MFH (46% vs. 65%; P = .034). Even after propensity score matching, these patterns persisted. Female NHPIs and NHPIs with a college degree were significantly more likely to report MFH, especially material and psychological MFH. CONCLUSIONS: A significantly higher proportion of elderly NHPI cancer survivors reported MFH, and this difference persisted even after propensity score matching, which warrants further investigation. Given the persistent mental health disparities between sexual minorities and the general population, social epidemiological research should address this disparity by investigating the differential impact of neighbourhood social environments across sexual orientations. There is growing evidence that neighbourhood cohesion, conceptualized as a sense of belonging and social connection, is an important social determinant of mental health in the general population, but little is known about its impact across sexual orientations. Using data from the UK household longitudinal study (2009-2018) including waves 1, 3, 6, and 9 (n= 52,903), this paper examined the longitudinal relationship between neighbourhood cohesion and mental health (using GHQ-12) across sexual orientations. A fixed-effect regression approach was taken to model the within-person change over time in GHQ predicted by neighbourhood cohesion with disaggregated analyses by gender and sexual orientation. Across all sexual orientations and genders, individuals who experienced an increase in neighbourhood cohesion also saw an improvement in their mental health over time. Moreover, the effect of neighbourhood cohesion on mental health over time differed by sexual orientation. Each 1-point increase in neighbourhood cohesion (on a 5-point scale) lead to mental health improvements of-0.8 GHQ score (95%CI-0.89 to-0.71) for heterosexual males at the lowest end, and up to-1.71 GHQ score (95%CI-2.31 to-1.11) for homosexual men at the highest end.

Restorative experience refers to an experience of psychophysical restoration from stress and mental fatigue during people-environment interactions. Previous research has documented the influence of neighbourhood physical environment on restorative experiences; however, it is still unclear whether neighbourhood social environment can affect restorative experiences. Applying a socioecological perspective, the current study aims to explore the effect of neighbourhood social cohesion on restorative experience and to examine whether people's need for restoration would moderate this effect. After manipulating the need for restoration in terms of antecedent attentional state, participants (N = 244) with and without the need for restoration read one of three experimental scenarios describing a neighbourhood with high, neutral, or low social cohesion. Participants reported their restorative experience pre-and post-exposure to neighbourhood social cohesion scenarios. Results showed that participants in the high social cohesion neighbourhood condition perceived more post-exposure restorative experience than did those in the neutral or low cohesion conditions. Restorative experience increased from pre-to post-exposure in the high social cohesion condition; the increase was greater for participants with the need for restoration. On the other hand, restorative experience decreased from pre-to post-exposure in the low social cohesion condition for participants without the need for restoration. These findings suggested that a socially cohesive neighbourhood could enhance individuals' restorative experiences and that the strength of this restorative benefit might depend on the need for restoration.
