Introduction One of the most commonly performed surgery by a general surgeon is inguinal hernia repair. There have been numerous open surgical techniques and two laparoscopic techniques described in the literature for the treatment of inguinal hernias. The treatment outcome of all these surgeries remains the same which is reducing the hernia and preventing recurrence. Our aim was to compare laparoscopic versus open inguinal hernia repair with emphasis on postoperative pain. Methods One hundred and twenty patients with unilateral primary inguinal hernias were randomly divided into two groups. Each group included 60 patients. Group one was treated by open Lichtenstein repair, while the second group was treated by laparoscopic transabdominal preperitoneal (TAPP) mesh repair. The two groups were compared to assess the duration of surgery, postoperative pain, duration of hospital stay, return to normal activity, and work. Results Laparoscopic TAPP repair was found to have a longer operative time as compared to Liechtenstein open repair. In terms of other parameters such as postoperative pain duration of hospital stay, return to normal activity, and work the laparoscopic group was superior. After a one-year follow-up, none of the patients had any chronic pain or evidence of hernia recurrence. Conclusion Laparoscopic TAPP has a clear advantage over the conventional Liechtenstein open surgery especially in terms of reduced early post postoperative pain and return to normal activity.

Postsecondary students confront many stressors that may contribute to the emergence of mental illness. 1 Longitudinal data suggest that rates of mental illnesses are increasing among Canadian postsecondary students, with more students reporting self-harm, suicidal ideation and suicide attempts over time. 1 When a student presenting at a university-affiliated health clinic is assessed to be at imminent risk of harm, legislative tools may be used to mandate emergent psychiatric assessment. In Ontario, Canada, under the province's Mental Health Act, 2 a "Form 1 Application by Physician for Psychiatric Assessment" can be completed to initiate this process. When a Form 1 is issued, students must be transferred to a designated hospital facility for evaluation.Although robust data are lacking, the annual number of student mental health transfers on some Ontario university campuses has increased substantially since 2014. 3 However, the processes for such transfers are an underresearched topic, part of a larger gap in research surrounding the conveyance of people experiencing a mental health crisis to hospital from communitybased points of care. [4][5][6][7][8] There is substantial heterogeneity in transfer processes when a Form 1 is completed for students pre-senting at university health clinics in Ontario. In particular, the involvement of police and use of physical restraints for transfers vary between institutions 8,9 and have been the subject of media scrutiny. 3,[10][11][12] These issues are important questions confronting health care professionals. 9 Some have argued for police involvement and use of restraints on the basis that they provide a type of safety to patients themselves, to health care professionals and to those transporting patients to hospital. 13 Opponents of routine police involvement and restraint use contend that such practices are traumatizing and stigmatizing, and perpetuate the criminalization of people with mental illness. Background: Minimally invasive surgery for groin hernia has expanded significantly over the last two decades and has demonstrated better outcomes in terms of pain and quality of life. A major contributing factor related to chronic post-operative inguinal pain (CPIP) is mesh fixation. An alternative to the standard fixation methods is the self-adhesive surgical mesh. Patients and Methods: Prospective data analysis was performed of all patients undergoing laparoscopic transabdominal pre-peritoneal (TAPP) inguinal hernia repair in a single centre for the period 1st January, 2022–15th December, 2022. A standardised surgical technique was used with a lightweight self-adhesive mesh without additional fixation. The analysis has encompassed early and late post-operative complications as well as the assessment of pain with an emphasis on CPIP. Results: The study enrolled 52 patients where a total number of 64 elective hernia repairs were performed: 92.2% (n = 59) primary and 7.8% (n = 5) recurrent. Fifty-one patients received post-operative follow-up: 100% at 1 month and 78.8% (n = 41) at 3 months. The incidence of early postoperative complications was 7.7% (n = 4): one patient developed a seroma, two patients – port site hematomas and one a transient subileus that were all managed conservatively. No patients suffered a recurrence. The average pain score according to the Visual Analogue Scale was 3.3 (0–8) at discharge, 0.6 (0–4) at 1 month and there was no incidence of CPIP after the 3rd month. Conclusion: Laparoscopic TAPP repair for inguinal hernia with a self-adhesive mesh is an adequate surgical technique with the potential to reduce CPIP, but more research is needed to evaluate this method.
