IntroductionBone density regulation is considered one of the systems affected by thyroid hormones, leading to low bone density that can result in pathologic fractures, including hip fractures. This review aimed to update clinicians and researchers about the current data regarding the relationship between hip fractures and thyroid disorders.MethodsEnglish papers were thoroughly searched in four main online databases of Scopus, Web of Science, PubMed, and Embase. Data extraction was done following two steps of screening/selection using distinct inclusion/exclusion criteria. This study used the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) checklist and the Newcastle-Ottawa Scale (NOS) as bias assessment.ResultsIn total, 19 articles were included in the research. The risk of hip fractures in women with differentiated thyroid cancer (DTC) is higher than hip fractures caused by osteoporosis. Men with hyperthyroidism and subclinical hyperthyroidism are at higher risk for hip fracture. Also, a decrease in serum thyroid stimulating hormone (TSH) may be associated with an increased risk of hip fracture.ConclusionReaching a consensus conclusion regarding the association between subclinical thyroid dysfunction and hip fracture is not feasible due to the heterogenicity of evidence; however, there may be a higher risk of fracture in individuals with subclinical hyperthyroidism.

O hipotireoidismo é uma condição endócrina comum, caracterizada pela produção insuficiente de hormônios tireoidianos. Essa disfunção afeta aproximadamente 4,6% da população mundial, com prevalência maior em mulheres e idosos. Objetivo: Revisar as etiologias, diagnóstico e o tratamento do hipotireoidismo. Metodologia: Trata-se de uma revisão bibliográfica, utilizando artigos presentes nas seguintes bases de dados: PubMed, Web of Science e Scopus Foram incluídos artigos originais e revisões sistemáticas em inglês e português publicadas entre 2013 e 2024, que abordassem sobre o hipotireoidismo. Após a análise, foram selecionados 31 artigos para a confecção dessa revisão bibliográfica. Resultados e Discussão: Entre as principais etiologias discutidas estão os distúrbios autoimunes, como a tireoidite de Hashimoto, o tratamento com iodo radioativo, deficiências nutricionais e os efeitos de medicamentos. Os exames laboratoriais, como a dosagem de TSH (hormônio estimulante da tireoide), T4 livre e anticorpos antitireoidianos, são fundamentais para o diagnóstico do hipotireoidismo. A terapia de reposição hormonal com levotiroxina, um hormônio sintético de tiroxina (T4), é a principal abordagem terapêutica para o hipotireoidismo primário. Conclusão: O hipotireoidismo é uma condição endócrina comum que afeta significativamente a qualidade de vida dos pacientes.

Objective: To study the mechanism of Liuwei Dihuang Pill (LWDP) in the treatment of hypothyroidism from the perspective of network pharmacology. Methods: The active ingredients of Liuwei Dihuang pill were screened by TCMSP platform database and the target prediction was carried out. GeneCards database was used to screen genes associated with hypothyroidism.The target of Liuwei Dihuang pill in treating hypothyroidism was obtained by Venny platform. The protein-protein interaction (PPI) network, gene ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis of these targets were performed to reveal the possible mechanism of Liuwei Dihuang pill in treating hypothyroidism. Results: A total of 28 active components and 189 targets were identified from Liuwei Dihuang Pills, and 4502 targets were identified from GeneCard database. The PPI network contains 124 nodes and 323 edges. 662 biological processes (BP), 72 cell components (CC) and 127 molecular functions (MF) were obtained by GO enrichment. A total of 149 signaling pathways were obtained by KEGG enrichment analysis, and the top 20 signaling pathways were visualized and analyzed.The signal pathways related to hypothyroidism include AGE-RAGE signal pathway, lipid and atherosclerosis signal pathway, HIF-1 signal pathway, PI3K-Akt signal pathway and MAPK signal pathway. Conclusion: The main components of Liuwei Dihuang pill in treating hypothyroidism are quercetin, kaempferol, β-sitosterol, diosgenin, kadsurenone, hederagenin, sitosterol and stigmasterol. The main targets of Liuwei Dihuang pill are MAPK1, TP53, TNF, AKT1 and EGFR, and they play a therapeutic role through AGE-RAGE, lipid and atherosclerosis, HIF-1, PI 3K-Akt and MAPK signal pathways.
