Purpose of the studyIron deficiency anaemia (IDA) is associated with increased morbidity and mortality in heart failure patients. The aim of our audit was to evaluate the current practice in diagnosis and assessment of IDA in patients admitted with heart failure.Study designWe conducted a retrospective audit of patients admitted to our hospital between January 2017 and June 2017 with a diagnosis of heart failure, and obtained data regarding each patient’s demographics and anaemic status. We also conducted a qualitative survey to assess healthcare professionals’ ability to diagnose IDA, and their knowledge of iron replacement in heart failure patients.ResultsOur audit identified 218 heart failure patients, nearly two-thirds (n=138, 63.3%) of which were anaemic. Of the 138 anaemic patients, only 40 had a full haematinic screen compared with 98 who had incomplete investigations (29% vs 71%, p=0.007). Iron studies were the most commonly performed haematinic investigation (n=87, 63%), and over half of these patients were iron deficient (n=49, 56.3%). Only 12 (24.5%) iron deficient patients were prescribed oral iron therapy, while 37 (75.5%) were left without iron replacement (X2=12.8, p=0.0003). Our survey demonstrated a lack of awareness among healthcare professionals with only 19.7% of participants being able to correctly define anaemia and 9.1% being aware of guidelines regarding treatment of IDA.ConclusionMany patients admitted to hospital with heart failure also have a concomitant diagnosis of anaemia. The aetiology of the underlying anaemia is often poorly investigated, and where IDA is identified it is poorly treated.

Введение. Анемия часто является сопутствующим заболеванием при хронический сердечной недостаточности (ХСН), которое снижает толерантность к физической нагрузке, качество жизни и ухудшает прогноз у пациентов. Такое влияние обуславливает актуальность изучения распространенности и патогенеза анемии и железодефицитных состояний при ХСН. Цель. Провести обзор литературы, иллюстрирующей современные данные о распространенности и патогенезе анемии и дефицита железа у пациентов с хронической сердечной недостаточностью. Материалы и методы. В данной работе представлен обзор литературы, посвященной распространенности и патогенезу анемии и дефицита железа у пациентов с хронической сердечной недостаточностью, исходя из проведенного поиска англоязычных статей в базе PubMed за последние 20 лет по ключевым словам: анемия, дефицит железа, хроническая сердечная недостаточность, патогенез, распространенность. Результаты. Частота анемии и дефицита железа у пациентов с ХСН в среднем составляет 30-70% и 30-50% соответственно. Были выделены 6 факторов, которые могут стать причиной анемии у пациентов с ХСН: дефицит железа, воспаление, уровень эритропоэтина, лекарственные препараты, гемодилюция, медуллярная дисфункция. Причины абсолютного дефицита железа анорексия, кахексия, нарушение абсорбции железа, функционального хроническое воспаление. Выводы. Лучшее понимание патогенеза этих состояний у пациентов с ХСН позволит разработать новые методы лечения. Intriduction. Anemia is often an associated disease in patients with chronic heart failure (CHF), which reduces exercise tolerance, quality of life, and worsens the prognosis in patients. This influence determines the relevance of knowledge of the prevalence and pathogenesis of these conditions in CHF. Purpose. Review of literature illustrating the current data about the prevalence and pathogenesis of anemia and iron deficiency in patients with chronic heart failure. Materials and methods. This work presents the review of literature about the prevalence and pathogenesis of anemia and iron deficiency in patients with chronic heart failure based on the search of English-language articles in the PubMed database for the last 20 years, by keywords - anemia, iron deficiency, chronic heart failure, pathogenesis, prevalence. Results. The frequency of anemia and iron deficiency in patients with CHF is about 30-70% and 30- 50%, respectively. Six factors can cause anemia in patients with CHF: iron deficiency, inflammation, erythropoietin levels, drugs, hemodilution, medullar dysfunction. The reasons for absolute iron deficiency are anorexia, cachexia, impaired absorption of iron, functional deficiency - chronic inflammation. Conclusion. Better understanding of the pathogenesis of these conditions in patients with CHF will let to develop the new methods of treatment.

El déficit de hierro sin anemia se encuentra presente en diversas patologías de la práctica clínica; sin embargo, se conoce poco sobre la importancia, las complicaciones y los beneficios de su tratamiento. Este artículo describe la evidencia actual sobre distintas vías fisiopatológicas, abordaje y tratamiento de las principales patologías relacionadas con la ferropenia sin anemia. Para ello se realizó una búsqueda completa y actualizada de la literatura científica en Medline, OVID, Lilacs, SciELO y EMBASE utilizando una estrategia definida con términos MeSH y no MeSH, limitado a español e inglés.
