Objective: To analyze the short-term clinical effect of radioactive iodine-125 seed implantation (125I-SI) for non-small cell lung cancer (NSCLC) patients and explore the correlation between the related metabolic parameters of pretreatment FDG PET-CT and the short-term efficacy of 125I-SI in patients with NSCLC, so as to provide useful reference for 125I-SI treatment. Methods: Retrospective analysis was performed on 46 patients with NSCLC, who received 125I-SI treatment for lung tumors in Tianjin First Central Hospital from January 2016 to December 2018. The correlation among D90, gender, pathological pattern, age, maximum tumor diameter, MTV, SUVmax, SUVpeak, SUVmean, TLG, High metabolic tumor cell ratio (HMR) and CEA with short-term efficacy of 125I-SI was analyzed by two independent-sample t-test, Mann-Whitney U-test or Chi-squared test and binary logistic regression. Results: One month after 125I-SI, 0 cases were CR, and 4 cases were PR, and the total effective rate was 8.7%. After 3 months, there were 2 cases of CR, and 25 cases of PR, with a total effective rate of 58.7%; after 6 months, there were 5 cases of CR, and 27 cases of PR, with a total effective rate of 69.6%. D90 (P = 0.028, OR:1.075,95%CI:1.008-1.147), MTV (P = 0.026, OR: 0.918,95%CI: 0.851-0.990), HMR (P = 0.020, OR: 0.003,95%CI: 0-0.407) were independent predictors for the short-term efficacy. The predictable accuracy of MTV was medium (AUC=0.781; cutoff value=44.58). However, the predictable accuracy of D90 and HMR was low, with both AUC of 0.650 (the cutoff =127.8Gy and the cutoff=0.27, respectively). Conclusion: 125-SI is an effective therapy with few complications in NSCLC patients. Small MTV, high D90 and low HRM were linked with better local control of 6 months after 125-SI.

Background: While hilar cholangiocarcinoma (HCCA) patients commonly undergo radioactive stent (RS) insertion treatment, the relative benefits of unilateral versus bilateral RS insertion procedures remain to be established. Accordingly, this study was designed to evaluate the relative safety and efficacy of percutaneous bilateral and unilateral RS insertion for patients with HCCA. Materials and Methods: In total, 126 HCCA patients that underwent unilateral (n=64) or bilateral (n=62) RS insertion from January 2017 - December 2021 were included in this analysis. Treatment efficacy and long-term outcomes were compared between groups. Results: The respective technical success rates in the unilateral and bilateral groups were 90.6% (58/64) and 93.5% (58/62) (P = 0.782). Both groups exhibited comparable medial postoperative bilirubin levels (100 vs. 99 μmol/L; P = 0.501), and restenosis occurred in 12 (20.7%) and 15 (25.9%) patients over the follow-up interval (P = 0.510). The median stent patency in the unilateral and bilateral groups was 189 and 210 days, respectively (P = 0.796), while the median overall survival interval was 222 and 229 days, respectively (P = 0.969). Comparable cholangitis (17.2% vs. 22.4%, P = 0.485) and cholecystitis (3.4% vs. 3.4%, P = 1.000) rates were also detected in these two groups. Conclusions: In summary, HCCA patients exhibit comparable efficacy when undergoing unilateral and bilateral radioactive stenting, suggesting that unilateral RS can be routinely performed owing to the simpler nature of this procedure.

Lung cancer is the leading cause of cancer mortality in the world. A significant proportion of patients with lung cancer are not candidates for surgery and must resort to other treatment alternatives. Rapid technological advancements in fields like interventional radiology have paved the way for valid treatment modalities like image-guided percutaneous and transarterial therapies for treatment of both primary and metastatic lung cancer. The rationale of ablative therapies relies on the fact that focused delivery of energy induces tumor destruction and pathological necrosis. Image-guided percutaneous thermal ablation therapies are established techniques in the local treatment of hepatic, renal, bone, thyroid, or uterine lesions. In the lung, the 3 main indications for lung ablation include local curative intent, a strategy to achieve a chemoholiday in oligometastatic disease, and recently, oligoprogressive disease. Transarterial therapies include a set of catheter-based treatments that involve delivering embolic and/or chemotherapeutic agents directed into the target tumor via the supplying arteries. This article provides a comprehensive review of the various techniques available and discusses their applications and associated complications in primary and metastatic lung cancer.
