Introduction of national guidelines for restrictive blood transfusion threshold for hip fracture patients––a consecutive cohort study based on complete follow-up in national databases

Abstract: BackgroundRandomized controlled trials have demonstrated that a restrictive red blood cell (RBC) transfusion strategy lowers transfusion frequency without affecting mortality. However, the external validity of these trials has not been tested in a large cohort. The purpose was to estimate the effect of introducing a National Clinical Guideline (NCG) for a restrictive hemoglobin transfusion threshold on transfusion frequency and mortality in hip fracture patients > 65 years old.MethodsA consecutive cohort study… Show more

“…The postoperative allogeneic transfusions rate was also evaluated. Criteria for the transfusion of blood products included a Hb level of less than 7.0 g/dL or a Hb level of less than 8.0 g/dL with clinical signs of symptomatic anemia, such as unexplained tachycardia, dyspnea, or hypotension [ 29 , 30 ]. In addition, all wound-related measures, including hematoma, wound dehiscence, oozing, suture granuloma, and superficial wound infection, during the follow-up period were included as wound complications [ 31 ].…”

“…The Hb level criterion used for transfusion is decreasing. In our study, the criterion for blood transfusion was strictly limited to cases with a Hb level <7 g/dL [ 15 , 16 , 17 ]. However, a Hb threshold of 8 g/dL was applied to patients with cardiovascular disease [ 18 ].…”

“…[2,3] RBC transfusions are associated with complications such as allergic reactions, bacterial or viral infections, circulatory overload, acute hemolysis, and acute lung injury. [4,5] All of these complications increase patient morbidity and mortality and patients’ financial burden. [6]…”
