Background Acute complete gastric volvulus is a rare and life-threatening disease, which is prone to gastric wall ischemia, perforation, and necrosis. If it is not treated by surgery in time, the mortality rate can range from 30 to 50%. Clinical presentations of acute gastric volvulus are atypical and often mimic other abdominal conditions such as gastritis, gastroesophageal reflux, gastric dilation, and pancreatitis. Imaging studies are crucial for diagnosis, with barium meal fluoroscopy being the primary modality for diagnosing gastric volvulus. Cases of acute gastric volvulus diagnosed by ultrasound are rarely reported. Case presentation We reported a rare case of acute gastric volvulus in a 4-year-old Chinese girl who presented with vomiting and abdominal pain. Ultrasound examination revealed the “whirlpool sign” in the cardia region, raising suspicion of gastric volvulus. Diagnosis was confirmed by X-ray barium meal fluoroscopy, which indicated left-sided diaphragmatic hernia and obstruction at the cardia region. Surgical intervention confirmed our suspicion of acute complete gastric volvulus combined with diaphragmatic hernia. Conclusion In this case, we reported an instance of acute complete gastric volvulus. Ultrasound revealed a “whirlpool sign” in the cardia, which is likely to be a key sign for the diagnosis of complete gastric volvulus.

Республиканское государственное бюджетное лечебно-профилактическое учреждение «Карачаево-Черкесская республиканская клиническая больница», ул. Гвардейская, д.1, Черкесск, Россия, 369000. АННОТАЦИЯ ЦЕЛЬ. Демонстрация клинического случая острого заворота желудка и краткий литературный обзор данного забо-левания. Материалы и методы. Ребёнок 12 лет, обратившийся по поводу острых болей в животе, многократной рвоты, с типичными признаками непроходимости верхнего отдела желудочно-кишечного тракта. Результаты. После проведённого клинического и инструментального обследования, ребёнок был оперирован в экстренном порядке. При лапаротомии было установлено наличие у больного острого вторичного заворота желудка с некрозом и перфорацией, на фоне истинной левосторонней диафрагмальной грыжи, тяжёлое поражение подже-лудочной железы. После ушивания дефекта желудка, пластики диафрагмы, была выполнена тщательная санация и дренирование брюшной полости. Послеоперационный период протекал тяжело в связи с осложнениями основного заболевания. В результате проведённого лечения больной был выписан в удовлетворительном состоянии. Заключение. Клинический случай подтверждает тяжесть данной нозологии, сопровождающейся серьёзными по-следствиями. Хотя в нашем случае больной выздоровел, диагностические ошибки и развивающиеся осложнения при остром завороте желудка нередко ведут к летальному исходу.Ключевые слова: заворот желудка, кишечная непроходимость, некроз, перфорация, детская хирургия I. To demonstrate a clinical case of acute gastric volvulus and to give a brief literary review of the disease. Materials and methods.A 12-year-old child who has applied for acute abdominal pain, repeated vomiting, with typical signs of obstruction of the upper gastrointestinal tract. Results. After the clinical and instrumental examination, the child was urgently operated. With laparotomy, the patient was diagnosed with acute secondary gastric volvulus with necrosis and perforation, against the background of a true left-sided diaphragmatic hernia, severe pancreatic lesions. The patient had a suturing of the stomach defect, diaphragm plastic, thorough sanitation and drainage of the abdominal cavity. The postoperative period was very difficult, due to complications of the underlying disease. As a result of the treatment, the patient was discharged in satisfactory condition. Conclusion.The clinical case confirms the severity of this nosology, accompanied by serious consequences. In our case, the patient recovered, but diagnostic errors and developing complications in acute gastric lavage often lead to death.
