Intraoperative Visible Bubbling of Air May Be the First Sign of Venous Air Embolism During Posterior Surgery for Scoliosis

Abstract: Visible air bubbling at the operative site may herald the onset of massive VAE during multilevel posterior spinal fusion and instrumentation. A prospective multicenter study using precordial Doppler, central venous catheter, and end-tidal CO2 is recommended to determine the true incidence of VAE in spinal deformity surgery and to evaluate monitoring and treatment methods.

“…Presence of visible bubbling at the operative site was usually the first clinical indication of VAE, but it can also happen without the visible bubbling. 9 There is no bubbling present in our case. A high index of suspicion could potentially save the life of the patient as a definitive diagnosis could be difficult.…”

“…After surgery, mercury spread out along the spinal canal and along the venous structure to the pulmonary artery. There have been many reported cases of air embolism during operation 4,8,10,14,15) . We assumed that not only air or gas but mercury as well could be transported by vessel, if it is contacted with exposed cancellous bone during bone works.…”

“…This occurs when there is a pressure gradient between the veins at the surgical site and the heart (Palmon et al ). Spontaneous intra‐operative VAE is a life‐threatening event and a fatal outcome was reported for 10 out of 22 humans affected during spinal surgery (Wills et al ). VAE has been associated with neurosurgery performed in the sitting position and also with lumbar puncture, arthroscopy and hip arthroplasty (Mirski et al ).…”
