Intraoperative mapping of language functions: a longitudinal neurolinguistic analysis

Abstract: Every attempt should be undertaken to preserve language-relevant areas intraoperatively, even when they are located within the tumor. New postoperative deficits resolve in the majority of patients, which may be a result of cortical mapping as well as functional reorganization.

“…Intra-operative direct electrical stimulation of the central nervous system provides a real-time, reliable, precise and safe method of functional mapping useful for all operations on lesions located in these areas [6][7][8][9][10][11][12] . “…Berger et al (21) and Duffau (22) applied DES to brain tumor surgeries, particularly low-grade glioma surgeries and concurrently carried out a large number of clinical studies in which the DES technology was applied to the location of subcortical pathways. All the aforementioned efforts have contributed outstandingly to the promotion of DES, making it one of the essential techniques in the functional area surgery of neurosurgery (23)(24)(25)(26). In 2002, Wang (Guangzhou, China) applied DES to the surgery of brain functional area lesions, and following four periods of nationwide study and training, this technology has gradually been accepted and promoted by neurosurgeons in China (11,12).…”

“…Учитывая ограниченность перечисленных методик, большинство нейрохирургов в настоящее время предлага-ют дополнительное использование интраоперационного электростимуляционного картирования функционально важных зон, проведение которого возможно под нарко-зом и под местной анестезией [26][27][28][29][30][31]. У пациентов с опу-холями, поражающими моторные зоны, оперативное вме-шательство проводится с картированием корковых полей под местной анестезией.…”
