Intraoperative intracardiac thrombus in liver transplantation: A 9‐year retrospective review focusing on treatment and outcomes

Abstract: This study characterizes incidence and outcomes surrounding intracardiac thrombosis (ICT) during liver transplantation over 9 years at a single center before and after the routine use of transesophageal echocardiography (TEE). Adult liver transplantation patients from 2011 to 2020 were divided into eras based on routine TEE use. ICTs were identified by querying anesthetic records for search terms. Descriptive statistics included counts and proportions for baseline recipient, donor, intraoperative, and postoper… Show more

“…Our study shows an even higher survival over a longer post‐operative period. A total of 91% of our patients receiving TPA for ICT were alive at six months after LT. By comparison, short‐term survival after major ICT has been reported as 33%–64% and the mortality of intraoperative cardiac arrest from all causes during LT approaches 31.6% 13,14 . Despite higher survival than previous reports, most patients experienced a prolonged post‐operative course.…”

“…13,14 One center showed short-term survival after ICT was 33.3% prior to adopting TEE use and improved to 64% after instituting TEE. 13 TEE is necessary for accurately diagnosing ICT as well as guiding supportive and thrombolytic treatment of ICT. 1,3,10,13,[15][16][17] We used TEE to diagnose all patients with ICT, to manage ICT treatment, and to confirm the resolution of ICT.…”
