Measuring intraocular pressure (IOP) is the cornerstone of a comprehensive glaucoma examination. In babies or small children, however, IOP measurements are problematic, cannot often be performed at the slit lamp, and sometimes require general anesthesia. Therefore, it is essential for an ophthalmologist who examines a pediatric patient to be aware of the different tonometers used in children, as well as the effects of central corneal thickness and anesthesia on IOP measurements. Goldmann applanation tonometry is the gold standard for IOP assessment. Most alternative tonometers tend to give higher IOP readings than the Goldmann applanation tonometer, and readings between different tonometers are often not interchangeable. Similar to Goldmann tonometry, many of these alternative tonometers are affected by central corneal thickness, with thicker corneas having artifactually high IOP readings and thinner corneas having artifactually lower IOP readings. Although various machines can be used to compensate for corneal factors (e.g., the dynamic contour tonometer and ocular response analyzer), it is important to be aware that certain ocular diseases can be associated with abnormal central corneal thickness values and that their IOP readings need to be interpreted accordingly. Because induction and anesthetics can affect IOP, office IOPs taken in awake patients are always the most accurate.

BACKGROUND The control of hemodynamic status is important because it can affect intraocular pressure (IOP) in patients undergoing eye surgery. We wanted to compare IOP changes in patients undergoing cataract surgery based on local anaesthesia and general anaesthesia with laryngeal mask and laryngoscopy types. METHODS In this double-blind non-randomized controlled trial; 170 patients 40-80 years old (ASA class I-II) candidates for elective cataract surgery enrolled to the study by convenience sampling method. After informed consent, patients were allocated to five groups A, B, C, D and E respectively. A) local anaesthesia, B) Macintosh laryngoscope, C) McCoy laryngoscope, D) video GlideScope and E) laryngeal mask airway. Systolic and diastolic blood pressures, mean arterial pressure, oxygen saturation and IOP were measured before, immediately and 5 minutes after airway intervention. IOP was measured with a Tono-Pen. Data was analysed by SPSS-16 using repeated measure ANOVA (Analysis of Variances), post hoc Tukey and chi-square tests. p-Value less than 0.05 was considered statistically significant. RESULTS 115 patients (67.6%) were female and 55 (32.4%) were male. In the local anaesthetic group, mean of IOP decreased significantly after airway intervention (p= 0.001). In laryngeal mask, Macintosh laryngoscope, McCoy laryngoscope and video GlideScope groups, IOP increased immediately and decreased 5 minutes after airway intervention (p = 0.001). Before the airway intervention, the mean of IOP in local anaesthetic group was higher than other groups (p= 0.012). Immediately and 5 minutes after air intervention, mean of IOP of the Macintosh laryngoscope was higher than other groups respectively (p< 0.001, p= 0.029). CONCLUSIONS Our results showed that Macintosh laryngoscopy has more effects on IOP in patients undergoing cataract surgery in comparison to other anaesthetic methods. Aim: Intraocular pressure (IOP) may decrease or increase during general anesthesia, depending on various factors. This study aimed to investigate the effects of the level of provider training period on post-intubation IOP values and hemodynamic response. Methods: This study was a cross-sectional observational study. Before inclusion in the study, informed consent was obtained from all participants. The study was approved by the local ethical committee. The study included 120 adult patients, both sexes, aged between 18 and 65, with physical statuses according to the American Society of Anesthesiologists (ASA) I or II, Mallampati score I. The research included 120 anesthesiologist resident doctors who received their training in our clinic. In this study, anesthesiology resident doctors were divided into three separate seniority groups (group 1, less than one-year residents in anesthesiology who had performed fewer than 10 intubations; group 2, one- to three-year residents; and group 3, more than three-year residents). After receiving a standard intravenous induction, direct laryngoscopy and endotracheal intubation techniques were performed. Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), and IOP were measured and recorded before pre-induction (T1), the first minute after induction (T2), and the first minute after laryngoscopy and intubation (T3). Results: There was no statistically significant difference (p > 0.05) between groups in the values of IOP, SBP, DBP, and HR measured at T1, T2, and T3. Measurements at T1, T2, and T3 were similar in all three groups. Comparisons within the groups revealed that IOP values at all measurement times (T1, T2, and T3) were different from each other in less than three-year resident groups. This difference was statistically significant (p < 0.001). The measurement values at T2 were the lowest and T3 were the highest in less than three-year resident groups. There was a significant increase in IOP after endotracheal intubation (T3) compared to baseline levels (T1) in less than three-year resident groups. IOP values at T2 were also significantly lower than the values at T1 and T3 (p < 0.001) in the more than three-year resident group (group 3). However, when we compared IOP measurements at T1 and T3 among themselves in the more than three-year resident group, no significant difference was found (p > 0.05). Conclusion: This study showed that endotracheal intubation in general anesthesia practice is performed most effectively by resident doctors with more than three years of anesthesiology training, without changing the IOP value.
