Intraocular antibody production in intraocular inflammation

Abstract: In patients with infectious uveitis, analysis of intraocular synthesis of specific antibodies is a valuable tool to establish the etiology rapidly and allows initiation of targeted antimicrobial treatment.

“…Aqueous humor and blood samples were collected simultaneously as previously reported [22,23,30,31,36]. A modified micro-ELISA technique (Enzygnost®, Dade Behring Marburg, Germany) was used to detect antibodies in serum and aqueous humor diluted to an IgG level of 1 mg/dl after the total IgG in the serum and aqueous were measured.…”

“…Aqueous humor samples were tested using micro-ELISA technique, as described elsewhere [12,18]. This technique allows detection of specific IgG even when little sampled material is available.…”

“…Kijlstra et al [14] in 1997 used PCR analysis of ocular fluids to define the diagnosis in 26 uveitis patients, but failed to find any antibody against herpes. More recently, we reported an intraocular antibodies synthesis in the diagnosis of 71 out of 89 uveitis patients [18]. So far, however, only one study has addressed the presence of antibodies in the aqueous humor in herpes keratitis [17].…”

“…But since the sensitivity of the laboratory tests is still unsatisfactory, the clinical picture and the course of the disease under therapy may be the only available pointers for a diagnosis. The strength or weakness of the clinical diagnosis may contribute to the variable sensitivity (i.e., confirmation frequency) of the results gleaned from combined aqueous humor and serum analyses, which ranges from 50 to 85% [7,20,29,35,42]. Moreover, the proportion of false-negatives and, worse still in terms of the treatment consequences, of falsepositives, is as yet unknown.…”
