SUMMARY:This study investigates the efficacy of chemonucleolysis using RGE in the treatment of cervical disk hernias in a small sample of patients who had cervical diskogenic or radicular pain secondary to disk herniations. Results were satisfactory in 89.5% patients, with no adverse events recorded during the procedure or after. The use of RGE shows promising results and might be a feasible and safe alternative in the treatment of cervical disk hernias.ABBREVIATIONS: CHU ϭ Centre Hospitalier Universitaire; IDH ϭ intervertebral disk hernias; RGE ϭ radiopaque gelified ethanol C hemonucleolysis is an efficient technique for the treatment of back pain; it consists of a percutaneous intradiskal injection that dissolves the nucleus pulposus and lowers intradiskal pressure. Several years ago, the most widely used substance for this was chymopapain, which had an efficacy of close to 80% in lumbar disk herniations and close to 85% in cervical disk hernias 1-3 but was withdrawn from the market due, in part, to anaphylactic reactions to this enzyme. 4 After this, the search for a new drug that could have the same efficacy as chymopapain but without the complications led to the use of pure ethanol 5 for chemonucleolysis with very good results and no allergic complications. The problem with pure ethanol was the high diffusibility of the liquid, which sometimes led to radicular burning pain after the procedure and, more important, to epidural leaks, which could be severe especially at the cervical level, where the spinal cord is closer to the dura.In treating venous angiomas, we had experience in using gelified pure ethanol, which had the same properties as liquid ethanol but could be better controlled at the time of injection and did not diffuse away from the site of injection.6,7 We previously published our experience with a similar radiopaque substance in the treatment of lumbar IDH with good results and no complications. 8 In this study, we investigate the potential efficacy of this substance in cervical disk hernias. TechniqueRGE is a mixture of pure ethanol and a thickening substance with tungsten powder. To investigate the efficacy of RGE, we set up a prospective preliminary study. Approval of the ethics committee of the hospital was obtained. Patients sent to be treated for a cervical IDH percutaneously, who had cervical diskogenic or radicular pain that did not resolve after the use of conventional therapy, were offered this alternative. An informed consent was obtained.For treatment purposes, we kept the basis of our original therapeutic concept used for many years: To reach the disk, we positioned the patient supine with a right anterolateral approach. After the treatment of the disk, we performed a concomitant injection of intraarticular steroids in the adjacent facet joints in all patients. 9Procedures were performed with local anesthetic, under surgical conditions and controlled digital fluoroscopy. Boyun ağrısı; bağlar, kaslar, bağ dokusu gibi yumuşak dokulardan kaynaklanacağı gibi, sinirlerden ve/ veya eklemlerden de kaynaklanabilir. En sık görülen boyun ağrısı nedeni, çeşitli etiyolojik faktörlere bağlı olarak ortaya çıkan yumuşak doku hasarıdır. Daha seyrek olarak, enfeksiyon ya da tümörler boyun ağ-rısına yol açabilir. Ayrıca boyun ağrısı, yansıyan ağrı şeklinde, yani sırtın üst bölümünün, omuzların, kolların problemlerinin yansıması olarak da gözlenebi-lir.[2] Dejeneratif ve inflamatuvar hastalıklar (kemik ve eklemlerdeki doğal yapının bozulması), özellikle 40 yaşından sonra boyun ağrılarının ortaya çıkma-sında önemli rol oynar. Romatoid artrit, osteoartrit, disk dejenerasyonu ve dar kanal bu bozukluklar arasındadır. [2] Servikal bölge, oldukça hareketlidir ve her yöne eğilebilir bir konumdadır. Birinci omurga olan atlas, mitolojide dünyayı havada tutan "Atlas" gibi başı G ünümüzde, özellikle kol kullanımının artması ve iş yerlerinde kullanılan bazı araçların (daktilo, bilgisayar, dokuma tezgahları, vb.) yaygınlaşması, boyun ağrılarının artmasına, boyun ve kol ağrılarının toplumların en yaygın sorunlarından biri halini almasına yol açmıştır. [1,2] Servikal omurga, kafa tabanından başlayan ve torakal (göğüs, sırt) omurgalarda (T1) sonlanan yedi omurgadan oluşmaktadır. Bu kemik yapılar, rahat hareketi sağlamak amacı ile bağlar (ligaman) ile birbirlerine bağlanmıştır. Bağların çevresindeki kaslar, destek sağlar ve harekete yardımcı olur. Boyun omurgaları, kendilerinden beklenmeyecek kadar yük kaldırmakla birlikte, tüm omurganın en korunmasız bölümüdür. Bu nedenle, herkes belli dönemlerde boyun ağrısı çek-miştir. Birçok kişi için boyun ağrısı, zamanla kendiliğinden geçen bir rahatsızlık iken, bunlardan bir kısmı için tıbbi tanı ve tedavi gerektiren bir sürece dönüşür. Kronik boyun ağrıları, hastaların en sık hekime başvuru nedenlerinden biridir. Günümüzde sedanter hayat ve genel olarak obezitenin artmış olması, kronik bel ve boyun ağrılarının sıklığında artmaya neden olmaktadır. Pek çok farklı yapıdan kaynaklanabilen kronik boyun ağrılarının başarılı tedavisi, öncelikle hastadan alınacak iyi bir anamnez ve fizik tedavinin yanında, görüntüleme yöntemlerinin kullanılmasını gerektirir. Ağrı nedeninin doğru saptanması, hedefe yönelik tedavilerin başarısını arttıran en önem-li parametredir. Boyun ağrılarının konservatif tedavilere yanıt vermemesi ve kronikleşmesi halinde, girişimsel yön-temlere başvurulur. ÖzOzon, üç oksijen atomundan oluşan, keskin kokulu, havadan ağır renks iz bir gazdır. Medikal ozon ise hastaya daima saf ozon ve saf oksijenin karışımı şeklinde belirli konsantrasyonlarda uygulanır. Pek çok hastalık türünde kullanılan ozon gazı, yan etkilerinin minimal oranda olması nedeniyle tercih sebebidir. Farklı medikal ozon uygulama şekilleri bulunmaktadır. Bel ve boyun ağrıları ise günlük pratikte polikliniğe başvuru nedenlerinin ilk sıralarında yer almaktadır. Lomber ve servikal disk hernileri bu ağrıların başlıca nedenlerinden biridir. Disk hernilerinde ozon tedavisi, klasik tedavi ve cerrahi tedavi ile beraber kullanılabilen tamamlayıcı bir tedavi yöntemidir. Ozon tedavisi, uygulama kolaylığı, tedavideki başarı oranları, düşük yan etki profili sayesinde ağrılı bel ve boyun hastalarında başarılı bir tedaviye yardımcı olur. Disk hernileri sonucu ortaya çıkan bel ve boyun ağrılarının yaşam kalitesini oldukça kötü yönde etkilediği ve toplumdaki sıklığı göz önünde bulundurulduğunda, medikal tedavi, fizik tedavi ve cerrahi tedaviye yardımcı bir yöntem olarak ozon tedavisi önemli bir yer tutmaktadır. Anahtar Kelimeler: Ozon tedavisi, boyun ağrısı, bel ağrısı Abstract Ozone consists of three oxygen atoms and is a colorless gas that is heavier than air with a pungent smell. Medical ozone is administered to the patient always in the form of a mixture of pure oxygen and pure ozone and in a certain concentration. The ozone gas, which can be used in many types of diseases, is preferred as its side effects are at minimal rate. There are different forms of medical ozone application. Cervical and lumbar pains are located in the first row of the reasons for applying to the clinics at daily practice. Lumbar and cervical disc herniation is one of the main causes of this pain. Ozone therapy is a complementary treatment which can be used along with conventional and surgical treatment for disc herniation. Ozone therapy allows a successful treatment for patients with cervical and lumbar pain due to its ease of implementation, high success rate in treatment and low side effect profile. Considering the negative effects of cervical and lumbar pain on quality of life and their prevalence in society, ozone therapy has an important place as an adjunctive therapy method to medical treatment, physical therapy and surgical treatment.
