Intracranial Complications before and after Endoscopic Skull Base Reconstruction

Abstract: ESBR offers an excellent long-term option in preventing subsequent ICC with low perioperative complications. ESBR is robust with delayed (>2weeks) CSF leakage occurring in only 1.9% regardless of etiology. The presence of identifiable risk factors for ICC may guide the surgeon in determining the urgency of ESBR.

“…This fact led, amongst others, to the introduction of the vascular pedicle intranasal mucoperiosteal flaps and to a consistently multilayered defect closure. The result was a reduction to less than 5% cerebrospinal fluid fistulas (in individual cases to no less than 20%) [70], [102], [182], [223], [248], [249], [250]. …”

“…In most cases this is decided individually, supporting a drainage in cases of large defects, heavy cerebrospinal fluid flow or increased cerebrospinal fluid pressure, history of radiotherapy or already preoperatively existing liquorrhea [182], [248]. An early drainage can help to relieve pressure variations within the area of transplantation during extubation [233].…”

“…10,19,35 While the incidence of CSF leaks after transsphenoidal surgery ranges from 1.5% to 10.3%, 43 rates associated with extended approaches for invasive skull base lesions have been reported to be as high as 19.4% in large series. 19 More recently, with the development of novel closure techniques, the leak rates have declined to less than 3%.…”
