Intracranial Aneurysms: Treatment with Bare Platinum Coils—Aneurysm Packing, Complex Coils, and Angiographic Recurrence

Abstract: More angiographic recurrences are detected over time. Complex coils do not augment aneurysm packing. Packing is not related to protection against recurrence.

“…An aneurysm recurrence rate between 20% and 30% after endovascular treatment has been reported [3,4,14,15]. The re-treatment rate in the International Subarachnoid Aneurysm Trial (ISAT) study group was 17.4% [14].…”

“…Results of these new devices, however, are sometimes disappointing, and it remains to be proved that modified coils provide better results. [24][25][26] One of the limitations in our study, which began in 1998, is the absence of an independent core laboratory to analyze our results. However, in each of the 5 participating centers in our study, the physician who collected and reviewed all of the aneurysms was independent of the treating physician.…”

“…A long-term follow-up is mandatory because there is a relevant rate of aneurysm recurrence of up to 30%, which does not always need retreatment, but, however, can be associated with a risk of rebleeding [3][4][5][6]. Many neurovascular centers perform a follow-up digital subtraction angiography (DSA) and a time-of-flight magnetic resonance angiography (TOF-MRA) at 3 to 6 months after treatment in order to continue with TOF-MRA alone, which has been shown to be sufficient in detecting recurrent aneurysms in the majority of the patients [7][8][9].…”
