Intracerebral hematoma after evacuation of chronic subdural hematoma

Abstract: A case of intracerebral hemorrhage occurring after evacuation of a chronic subdural hematoma is presented. The mechanism of this complication is not clear. However, hemorrhage into previously undetected areas of contusion, damage to cerebral vasculature secondary to rapid perioperative parenchymal shift, and sudden increase in cerebral blood flow with focal disruption of autoregulation have been postulated as causes. Although this is a rare complication after decompressive operation for chronic subdural hemato… Show more

“…[9] Symptoms of the ICH appeared in immediate postoperative period[410111213] or several days after the event. [2451114] In all the cases the hematoma developed in the ipsilateral hemisphere, but in our case the hematomas were multiple and appeared bilaterally with large part on the contralateral side. This complication usually lead to the poor outcome in these patients[45101112] with a fatal outcome reported in one-third of the patients, with another third severely disabled.…”

“…Of the patients with recurrence, 23 (3.6%) were older than 60 when cerebral atrophy was most pronounced. Other surgical complications included tension pneumocephalus [47] or the development of intracranial acute hematomas [48,49].…”
