Intra-articular Osteoid Osteoma of the Proximal Ulna

Abstract: Osteoid osteoma is a common benign osteogenic bone neoplasm characterized histologically by increased osteoid tissue formation with an intracortical nidus surrounded by cortical thickening and vascular fibrous stroma and sclerosis. The clinical presentation classically includes severe nocturnal pain that is improved with nonsteroidal anti-inflammatory drugs. Younger men (second and third decades) have the highest incidence, with the most frequent sites of involvement being the long bones or axial skeleton. Ost… Show more

“…O steoid osteoma is a painful bone tumor, and has been infrequently described at various locations within the elbow joint. [1][2][3][4] The lesion is usually associated with disabling pain and occasionally may result in progressive stiffness of the elbow. 5 Radiofrequency ablation is usually the treatment of choice; however, arthroscopic excision has been described as an alternate management option and can be combined with arthroscopic elbow adhesiolysis for restoration of motion.…”

“…OO is a benign lesion comprising 10–12% of bone tumors [ 5 ] with a male predominance mostly in their second and third decade [ 3 , 6 ]. The more common diaphyseal tumors present with classical night pain relieved by salicylate/NSAIDs and characteristic X-ray finding of a radiolucent nidus with typical surrounding profound sclerosis [ 7 , 8 ].…”

“…The more common diaphyseal tumors present with classical night pain relieved by salicylate/NSAIDs and characteristic X-ray finding of a radiolucent nidus with typical surrounding profound sclerosis [ 7 , 8 ]. High levels of prostaglandin E2 in the nidus have been attributed to the classical night pain which relieves on prostaglandin inhibitors [ 6 ]. Surgical excision gives best results in symptomatic patients, while percutaneous ablation is a safe alternative.…”

“…Our young patient represents such a case, whose X-rays revealed a reactive periostitis lesion that could be mistaken for any other kind of tumor. In order to set the right diagnosis apart from the X-rays, a CT scan and a three-phase bone scan can be very useful [ 7 ]. Even with these examinations, identification of the tumor is difficult.…”
