Background-: Stoke is characterized as rapid clinical signs of focal (or global) impairment of brain function, with symptoms that last longer than one day or lead to death, with no other apparent cause than vascular origin. Globally, Cerebrovascular(CVE)damage is the second-biggest root cause of death and the third-largest cause of disability. This is 1 in 19 deaths. Stroke is a significant source of injury in the long term and is more disabled than fatal. This protocol has been created that describes the design of experimental study to evaluate the combined effect of Neuromuscular Electrical Stimulation(NMES) with Proprioceptive Neuromuscular Facilitation(PNF) to enhance the upper-extremity functions in hemiplegic stroke patients.Methods-The participants (n=26) will be recruited in the study suffering from stroke and meeting the inclusion criteria. Two groups will be formed such that patients in group A will be treated with conventional therapy and PNF group B will be treated with PNF and NMES in alongwith conventional therapy. The protocol will cover 6 weeks of treatment. Regular assessment at 0,3 rd and 6 th week will be carried out. In the rehabilitation period, we will evaluate upper extremity function, activities of daily living(ADLs), mental status examination at regular interval during the rehab.Our outcome measures will be-Glasgow coma scale (GCS), Mini-Mental Scale Examination (MMSE), Modified Ashworth Scale (MAS), Modified BarthelIndex(MBI) and Action Research Arm Test(ARAT).Discussion-Efficacy of the intervention will be evaluated by analyzing the upper extremity function using Action Research Arm Test (ARAT) and level of disability by using Modified Barthel Index Scale. The result of the study will significantly provide affirmation on using combination therapy of PNF with NMES in stroke patients.

The purpose of the study was to determine the prognostic role of finger extension and to assess the function of the upper extremity in patients with stroke. Materials and methods. The study was performed on the basis of the Medical Center "Expert Health" and clinical units of Petro Mohyla Black Sea National University, Mykolaiv, Ukraine during 2020-2021. 89 patients who underwent acute cerebrovascular accident by ischemic type with paresis of the upper extremity were examined. All patients were examined in accordance with current clinical protocols in 3 and 6 months after undergoing stroke. Additionally, patients underwent a standard hand function test – Action Research Arm Test, as well as an extended test – Action Research Arm Test +in its own modification (IsoTren LTD). Statistical processing was performed by methods of analysis of variance and correlation using Statisica 13.0 software (TIBCO, USA). Results and discussion. When assessing the functional status of patients, the average values of Barthel Index were 65.4±1.4 points, which corresponded to 2.7±0.3 points by modified Rankin Scale. The main difficulties were observed in such skills as writing (d170), use of means of communication (landline and mobile phone, computer, gadgets – d360), fine motor movements (d440), cooking (d630), eating (d550), washing (d510), personal hygiene (d520) and toilet use (d530), etc. When evaluated according to the standard Action Research Arm Test 3 months after the stroke, the average score was 39.9±1.5, according to the subtest of the Extensors’ Function Assessment Test – 8.3±0.4 points, which corresponds to the overall score for Action Research Arm Test +of 48.1±1.4 points. After 6 months, the score of the Action Research Arm Test 3 months after the stroke, the average score was 43.4±1.2 points ( = +8.8%), according to the subtest of the Extensors’ Function Assessment Test – 9.9±0.4 points ( = +19.3%), which corresponds to the overall score on Action Research Arm Test +of 53.2±1.3 points ( = +10.6%). Correlation analysis showed that the results of the subtest of the Extensors’ Function Assessment Test and Action Research Arm Test significantly correlate with each other (r = 0.72 p <0.01). The developed original test to assess the function of extensors has a specificity of 95.6% with a sensitivity of 98.5% (J = 0.95). This estimate corresponds to LR += 24.75, which allows us to assess the prognostic value of finger extension as very high. When assessing the internal consistency of the standard test Action Research Arm Test and Action Research Arm Test +, it was found that Cronbach = 0.87, which indicates a high degree of consistency of the tests. Conclusion. It is advisable to use the extensor function assessment test together with the standard Action Research Arm Test (modified Action Research Arm Test +)

Introducción: ninguna escala simple, por diseño, es lo suficientemente abarcadora para evaluar de forma integral la función motora de pacientes con enfermedad cerebrovascular (ECV); sin embargo, es posible la selección de una batería de ellas para controlar las distintas etapas del proceso de rehabilitación. El objetivo de este estudio fue identificar los criterios que sirven de referencia para la construcción de un instrumento de evaluación funcional en pacientes con ecv. Materiales y métodos: se realizó una búsqueda bibliográfica en las bases de datos PubMed, Rehabilitation Measures Database, ebsco y Medline, donde se incluyeron escalas de evaluación funcional dirigidas al control de la postura, marcha, equilibrio, tono muscular, miembros superiores e inferiores y actividades de la vida diaria. Resultados: para la construcción de un instrumento de evaluación funcional se identificaron un total de trece criterios de referencia agrupados en: 1) criterios psicométricos y 2) criterios de diseño y aplicación. Utilizando estos criterios, se seleccionaron las escalas: Barthel Index, Action Research Arm Test, Ashworth Scale y 10 Meter Walk Test. Conclusión: las escalas seleccionadas brindan información sobre cuatro aspectos relevantes para la evaluación funcional y la práctica terapéutica: autocuidado, función motora, tono muscular y rendimiento físico, estrechamente relacionados con las principales áreas de trabajo durante el proceso de rehabilitación en pacientes con ECV. Los recursos materiales necesarios para su aplicación son mínimos y el tiempo estimado, a partir de la integración de sus tiempos parciales, no sobrepasa los 30 minutos.
