The clinical course of intensive care unit (ICU) patients may be complicated by a large spectrum of lower respiratory tract infections (LRTI), defined by specific epidemiological, clinical and microbiological aspects.A European network for ICU-related respiratory infections (ENIRRIs), supported by the European Respiratory Society, has been recently established, with the aim at studying all respiratory tract infective episodes except community-acquired ones. A multicentre, observational study is in progress, enrolling more than 1000 patients fulfilling the clinical, biochemical and radiological findings consistent with a LRTI. This article describes the methodology of this study. A specific interest is the clinical impact of non-ICU-acquired nosocomial pneumonia requiring ICU admission, non-ventilator-associated LRTIs occurring in the ICU, and ventilator-associated tracheobronchitis. The clinical meaning of microbiologically negative infectious episodes and specific details on antibiotic administration modalities, dosages and duration are also highlighted. Recently released guidelines address many unresolved questions which might be answered by such large-scale observational investigations. In light of the paucity of data regarding such topics, new interesting information is expected to be obtained from our network research activities, contributing to optimisation of care for critically ill patients in the ICU.

ResumenLas infecciones respiratorias agudas (IRA) constituyen uno de los motivos de consulta más frecuentes en los Servicios de Urgencias (SU) y se encuentran entre las primeras causas de morbimortalidad en los países en vías de desarrollo, según la Organización Mundial de la Salud (OMS). De manera general, se pueden definir como un deterioro clínico agudo caracterizado por la presencia de tos, expectoración, disnea, fiebre, cambios en las características del esputo y alteraciones radiológicas; todas estas manifestaciones pueden comprometer de manera significativa la salud de los pacientes. Realizar un buen diagnóstico y tratamiento de las IRA es primordial en la práctica diaria, tanto en Atención Primaria como a nivel hospitalario, para evitar complicaciones y mortalidad relacionadas con estas enfermedades. En esta actualización, hablaremos de las infecciones respiratorias más frecuentes en el SU, la bronquitis aguda, las agudizaciones de la EPOC o bronquiectasias y las neumonías. Abstract Update on respiratory infections in the emergency departmentAcute respiratory infections (ARI) are among leading causes of emergency department (ER) consultancy and, according with World Health Organization (WHO) is one of the first causes of morbimortality in developing countries. ARI is defined as an acute clinical deterioration characterized by cough, spitting, dyspnea, fever and sputum and radiological changes, which can endanger patient's health significantly. In order to prevent clinical complications and death, proper diagnosis and treatment are mandatory in both Primary and Hospital Care. Die zweite Jahreshäl e, die sich nun schon dem Ende zuneigt, ist für unseren Kollegenkreis durch die großen europäischen Kongresse interessant. Da gibt es den der ERS (European Respiratory Society), der ESMO (European Society for Medical Oncology) und der ESC (European Society of Cardiology), um nur die wichtigsten für das pneumologische Fachgebiet zu nennen. Die Fülle der sich zum Teil überlappenden klinischen und grundlagenwissenscha lichen Informationen ist auch in diesem Jahr überwältigend. Was davon ist hier für die Pneumologie als besonders interessant herauszu schen? Asthma bronchialeHighlights beim Asthma bronchiale sind Arbeiten zur zielgerichteten entzündungshemmenden erapie. Damit geht nun auch die Pneumologie den Weg, den lange vorher schon die Rheumatologie und die Onkologie beschritten. Neue Antikör-per hemmen spezi sch die durch eosinophile Granulozyten oder Immunglobulin E (IgE) dominierte Entzündung. Der Effekt auf die Lungenfunktion ist zwar eher marginal, der auf die Senkung der Exazerbationsrate aber deutlich. Dies ist auch vor dem Hintergrund der aktuellen Empfehlungen der GINA (global initiative for asthma) von Bedeutung, in der die Graduierung nach lungenfunktionsbezogenen Schweregraden zugunsten der Asthmakontrolle völlig fallen gelassen wurde.
