Intermediate care for older people - an implementation

Abstract: Intermediate care services are being developed in England and elsewhere to provide an integrated health and social care model for older people with sub-acute illness and/or needing rehabilitation. A more rapid implementation of these services might be achieved if the practical service development issues were better understood.
