In 287 consecutively intracapsular cataract extracted eyes with a Federow intraocular lens implant (IOL) and 290 eyes with consecutively performed intracapsular cataract extractions (ICCE) the final visual outcome and complications were examined in a retrospective study. The average observation time was 31 months in the IOL group and 37 months in the ICCE group. Seventy-five per cent in the IOL group and 77% in the ICCE group achieved a visual acuity greater than or equal to 0.5. The main cause of a visual acuity less than 0.5 was maculopathy pre-operatively recognized. A visual acuity less than 0.5 due to post-operative complications occurred more frequently in the IOL group (28 eyes) compared with the ICCE group (13 eyes) - P less than 0.01. Twelve intraocular implants (4.3%) had to be removed. The most important cause was dislocation of the IOL (9 eyes). During our first 12 months of IOL implantation 8 dislocations occurred out of 56 implanted eyes (14.3%). The remaining 6 dislocations occurred among the last 231 IOL eyes (2.6%). It is concluded that complications are few, but often serious in intraocular lens implant surgery. Careful pre-operative examination and surgical experience are mandatory.

Sixty-four patients were consecutively operated for senile cataract with implantation of the semiflexible McGhan/3M, style 70, anterior chamber lens. Intraocular pressure (IOP) was measured one day prior to the operation, 6 days and 3-8 months (mean 4 months) after the operation, and gonioscopy was performed one day prior to and 4 months after the operation. A significant fall in IOP was found in the operated eye 6 days after surgery. No significant change in IOP was found 4 months after surgery, neither in the operated eye nor in the non-operated fellow eye. Iris tucking seemed to have no influence on the post-operative tension, while a significant rise in IOP was found in 13 eyes with adhesions between the iris and lens haptics as the only abnormal gonioscopic finding.

ÖzetAmaç: Erken ve geç dönem göz içi lens (GİL) dislokasyonlarında 23 gauge (23 G) pars plana vitrektominin (PPV) etkinlik ve güvenliğini değerlendimek. Ge reç ve Yön tem: Ocak 2009-Haziran 2011 tarihleri arasında GİL dislokasyonu nedeniyle 23 G PPV uygulanan 17 hastanın dosyaları geriye dönük olarak incelendi. Tüm olgularımıza ayrıntılı oftalmolojik muayene yapılıp ameliyat esnasında ve sonrasında görülen komplikasyonlar kaydedildi. Sonuçlar: Çalışmaya alınan olguların yaş ortalaması 60,8 (8-82) yıl, ortalama takip süresi 9,0 (4-22) aydı. Katarakt ameliyatı sonrası geçen süre değişken olup ortalama 28,3 (0-80) ay idi. Olgularımızın 4'ünde travma öyküsü, 5'inde psödoeksfoliasyon sendromu ve 2 olgumuzda patolojik myopi mevcuttu. Olgularımızın 9'unda (%52,9) disloke olan GİL sulkusa yerleştirilirken, 2 (%11,7) olguda GİL çıkarılıp olgular afak bırakıldı. Diğer 6 olgunun 2'sinde (%11,7) disloke olan GİL'i skleral fiksasyon lensi ile değiştirilirken, 4 (%23,5) olguda GİL iris kıskaçlı lens ile değiştirildi. On iki (%70,5) olgumuzda EİDGK iki sıra veya daha fazla artarken, 1(%5,8) olgumuzda görme aynı düzeyde kaldı. Olgularımızın 2 (%11,7) tanesinde ise görme azaldı. Ayrıca olgularımızın birinde ilaçla kontrol altına alınabilen göz içi basınç yüksekliği gelişirken 1 olguda 2 gün süren hipotoni izlendi. Ma te ri al and Met hod: In this retrospective study, the medical records of 17 patients who underwent 23 G PPV treatment for dislocated IOL between January 2009 and June 2011 were analyzed. Detailed ophthalmologic examination was performed in all patients; intraoperative and postoperative complications were recorded.Re sults: In this study, mean follow-up time was 9.0 (4-22) months and mean age was 60.8 (8-82) years. Mean time after cataract surgery was variable (mean of 28.3 (0-80) months). Four patients had trauma history, five patients had pseudoexfoliation syndrome, and two patients had pathologic myopia. Dislocated IOL was placed into the sulcus in nine patients (52.9%). In two cases (11.7%), the IOLs were removed, and the patients were left aphakic. In two cases out of other 6 (11.7%), the dislocated IOL was changed with scleral fixation lens, whereas in the other four cases (23.5%), IOL was changed with iris claw lens. One patient (5.8%) remained at the same level of vision, while twelve (70.5%) cases increased two or more lines in BCVA. The vision was decreased in two cases.
