Introduction: The objective of this systematic review was to answer the question: Does the intraradicular post transfixed in the dental crown increases the fracture resistance of weakened and directly restored teeth? Methods: Electronic databases (MEDLINE/PubMed, LILACS, SCOPUS, EMBASE, Scientific Electronic Library Online - SCIELO, and Central Register of Controlled Trials - CENTRAL) were searched until March 2021, without language or year restriction. Grey literature was also searched through Google scholar and OpenGrey repository. Only in vitro stud-ies were included that evaluated the influence of the use of intraradicular post trans-fixed in the crown in the buccopalatal/lingual direction in the fracture resistance of the dental crown. Relevant results were summarized and evaluated. The risk of bias was also assessed in the studies. Results: Initial screening of databases resulted in 249 studies, of which 109 were excluded for being duplicates. Of 140 eligible pa-pers, fourteen studies met the inclusion criteria and were selected for full-text read-ing. Of these, two studies were excluded for not having access to the full article. All selected articles were classified as low risk of bias. Conclusion: Based on the studies, it is possible to conclude that the use of a transfixed post in the crown increases the fracture resistance of weakened and directly restored teeth.

A restauração de dentes tratados endodonticamente com grandes perdas de estrutura dentária é um verdadeiro desafio para a Odontologia, visto a necessidade de utilização de materiais com capacidade de ampliar o suporte coronário e impedir a propagação de fraturas. Recentemente, uma mudança sobre o uso de retentores intrarradiculares vem surgindo, criando um novo conceito “no post”, a partir da utilização de compósitos resinosos reforçados por fibras de vidro. O objetivo desta revisão consiste em explorar na literatura se o material, representado pela EverX Posterior (GC), apresenta alguma vantagem de uso em comparação com os outros materiais já descritos para a restauração de dentes tratados endodonticamente. A busca foi realizada nas principais bases de dados, PubMed, Scopus e Web of Science, resultando em 163 artigos. Após a exclusão de duplicatas e leitura na íntegra, 14 artigos in vitro e 1 artigo clínico foram incluídos na versão final de análise. Com relação aos dados encontrados, o novo biomaterial apresentou maior resistência à fratura em dentes posteriores na maioria dos estudos avaliados, exceto em comparação com as restaurações indiretas, principalmente do tipo endocrown. Em comparação com as restaurações diretas, a EverX Posterior, em “bicamada”, funcionou como um reforço para distribuição de forças e com propriedades melhoradas na presença de ranhuras retentivas. Já para os retentores intrarradiculares, o novo biomaterial também garantiu uma melhora do comportamento mecânico, semelhante à comparação feita para as fibras de polietileno e de vidro. Desse modo, novos estudos clínicos devem ser feitos para comprovar as considerações sobre essa filosofia.

The restoration of endodontically treated teeth with great loss of tooth structure is a real challenge for dentistry, since it requires the use of materials capable of extending the coronal support and preventing the propagation of fractures. Recently, a change in the use of intraradicular retainers has emerged, creating a new "no post" concept, based on the use of glass fiber reinforced resin composites. The aim of this review is to explore in the literature whether the material, represented by EverX Posterior (GC), presents any advantage of use in comparison with other materials already described for the restoration of endodontically treated teeth. The search was conducted in the main databases, PubMed, Scopus and Web of Science, resulting in 163 articles. After exclusion of duplicates and full-text reading, 14 in vitro articles and 1 clinical article were included in the final analysis version. With regard to the data found, the new biomaterial showed higher fracture resistance in posterior teeth in most of the studies evaluated, except in comparison with indirect restorations, especially the endocrown type. In comparison with direct restorations, EverX Posterior, in a "bilayer", worked as a reinforcement for force distribution and with improved properties in the presence of retentive grooves. For the intraradicular retainers, the new biomaterial also guaranteed an improved mechanical behavior, similar to the comparison made for polyethylene and glass fibers. Thus, further clinical studies should be carried out to confirm the considerations about this philosophy.
