Вackground In the last two decades, health care systems (HCS) in the European countries have faced global challenges and have undergone structural changes with the focus on early disease prevention, strengthening primary care, changing the role of hospitals, etc. Russia has inherited the Semashko model from the USSR with dominance of inpatient care, and has been looking for the ways to improve the structure of service delivery. This paper compares the complex of structural changes in the Russian and the European HCS. Methods We address major developments in four main areas of medical care delivery: preventive activities, primary care, inpatient care, long-term care. Our focus is on the changes in the organizational structure and activities of health care providers, and in their interaction to improve service delivery. To describe the ongoing changes, we use both qualitative characteristics and quantitative indicators. We extracted the relevant data from the national and international databases and reports and calculated secondary estimates. We also used data from our survey of physicians and interviews with top managers in medical care system. Results The main trends of structural changes in Russia HCS are similar to the changes in most EU countries. The prevention and the early detection of diseases have developed intensively. The reduction in hospital bed capacity and inpatient care utilization has been accompanied by a decrease in the average length of hospital stay. Russia has followed the European trend of service delivery concentration in hospital-physician complexes, while the increase in the average size of hospitals is even more substantial. However, distinctions in health care delivery organization in Russia are still significant. Changes in primary care are much less pronounced, the system remains hospital centered. Russia lags behind the European leaders in terms of horizontal ties between providers. The reasons for inadequate structural changes are rooted in the governance of service delivery. Conclusion The structural transformations must be intensified with the focus on strengthening primary care, further integration of care, and development of new organizational structures that mitigate the dependence on inpatient care.

Aim. To analyze the readiness of the professional medical community for changes in the classification and implementation of novel approaches to heart failure (HF) treatment based on an online survey.Material and methods. Anonymous online survey of physicians on the official website of the Russian Society of Cardiology (www.scardio.ru) "Chronic heart failure. Unsolved issues" was held from September 30 to October 4, 2023. The survey involved 1015 doctors with an average length of work >10 years, mainly cardiologists (n=810, 83,2%).Results. The majority (n=591, 58,2%) of respondents considered it necessary to identify the HF prestage in the classification, similar to prediabetes or prehypertension, while only 2,2% (n=22) of respondents did not consider it necessary to indicate the HF risk in the classification. Simplifying the HF classification based on the left ventricular ejection fraction (LVEF) values with the identification of only two gradations of LVEF <50% and LVEF ³50% was supported by 60,3% (n=612) of specialists.The majority (80,8%) of respondents supported increasing the time of admission of a patient with HF to 30 minutes to ensure the healthcare quality. According to the survey, only 32% of respondents have the ability to determine the brain natriuretic peptide level in patients with HF without restrictions, which indicates insufficient material security for the implementation of clinical guidelines in practice. Quadruple therapy is prescribed to every patient with HF by only 37% of respondents who took part in the survey, which may indicate clinical inertia regarding following the guidelines.Conclusion. In general, the survey demonstrated high awareness of physicians about the problem of HF and readiness to assess the risk and diagnose HF at preclinical stages, as well as a positive attitude towards changing the classification.

Aim. To evaluate the importance of certain aspects of research-to-practice conferences, organized by the Department of General Practice of Pirogov Russian National Research Medical University, as well as its contribution to preparing students for future professional activities within the outpatient healthcare and the impact of these events on the interest of participants and speakers for research work.Material and methods. An anonymous online survey included 37 participants in the discussed conferences regarding the respondents’ motivation to attend conferences, assessment of declared subject coverage, the practical applicability of the knowledge gained and other aspects.Results. The most important motivating factors for conference participation were interest in outpatient healthcare (52,4% for the conference "Internal diseases at the prehospital stage" and 87,5% for the conference "A multimorbid patient — continuity between hospital and clinic"), analysis of interesting cases (66,7% and 75%, respectively) and multidisciplinary focus of activities (47,6% and 68,8%). An important aspect was the coverage of current problems of practical healthcare. The most important results were considered by the majority of participants to be expanding their medical horizons (85,7% and 87,5%) and gaining new knowledge on the declared subject (90,5% and 75%). Almost half of the respondents (52,4% and 56,3%) improved their interest in scientific activities as a result of visiting the conferences.Conclusion. Conducting research-to-practice conferences for students, residents and postgraduates serves as a platform for increasing the knowledge and mastering professional competencies. The results of a survey of conference participants and speakers indicate the relevance of involving students in conferences and the numerous benefits they receive, which are discussed in detail in this article.
